Description
Combined Synopsis/Solicitation for Autoimmune Testing/HIV-1 Western Blot for Air Force Institute for Operational Health/SDE (i) This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. (ii) The solicitation reference number is F1JEAC7183B002 and is issued as a request for proposal (RFP). (iii) This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-18. (iv) This solicitation is issued under full and open competition with no small business set-aside. (v) This requirement will consist of Contract Line Item Numbers (CLINs) which will reference a price list for kits (reagents) necessary to perform the tests listed in para 2.7 and 2.8 of the Performance Work Statement (PWS). CLIN 0001 will reference a price list for Auto-Immune testing listed in para 2.7, while CLIN 0002 will reference a price list for HIV-1 Western Blot listed in para 2.8. The cost for the use of the platform to perform those tests and necessary support services shall be included in the costs for the kits. (vi) The description of requirements for this service are outlined in the attached PWS dated 02 August 2007. (vii) This BPA will be in effect for four years beginning 31 August 2007. The platforms and all kits will be delivered FOB to Air Force Institute for Operational Health (AFIOH), Epidemiology Surveillance Division (SDE) Bldg. 930, Brooks, City-Base TX 78235-5132 . (viii) FAR 52.212-1, Instructions to Offerors -- Commercial, applies to this acquisition. (ix) FAR 52.212-2, Evaluation applies. The evaluation factors that constitute para (a) of the clause are listed following the attached PWS. (x) Offerrors should submit a completed copy of the provision at 52.212-3, Offeror Representations and Certifications -- Commercial Items, with their offer. (xi) FAR 52.212-4, Contract Terms and Conditions -- Commercial Items applies. (xii) FAR 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders -- Commercial Items, is applicable. The following clauses are checked under para b. of 52.212-5 and considered applicable to this solicitation: 16-21, 35. This BPA can be cancelled by the Government upon a 30-day written notice to the BPA Holder or by expiration date of the BPA, whichever occurs first. This BPA can be cancelled by the BPA holder upon a 90-day written notice to the Government or by expiration of the BPA, whichever occurs first. ( xv) Not Applicable.
