Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation is issued as a Request for Quote W911XK-07-T-0109. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circu lar 2005-18. Acquisition is 100% small business set-aside. The NAICS Code is 561730 and the small business size standard is $6,500,000.00. The scope of work consists of the clearing and grubbing of trees and woody vegetation located at eight separate U.S. Army Corps of Engineers Dam locations along the Lower Fox River. The dams are Menasha Dam, Appleton Upper Dam, Appleton Lower Dam, Cedars Dam, Little Chute Dam, Kaukauna Dam, Little Kaukauna Dam and DePere Dam. The eight Federal Dams are located on the Lo wer Fox River in Wisconsin. The dams are located along 60 miles of the Lower Fox River, from Menasha Dam, located at the mouth of Lake Winnebago in Menasha, Wisconsin, to Depere Dam located in Depere, Wisconsin (See Figure 1). The required work areas are shown in Figures 2  10. [Note: Figure 8 intentionally omitted]. Trees and woody vegetation do not cover the entire area shown on the figures. The contractor is required to make a site visit to all nine sites prior to submitting a proposal. The specific areas to be cleared will be located and marked in the field by USACE personnel prior to site visits. The work consists of mobilizing all necessary equipment, materials, and manpower to the site, MARINE PLANT MAY BE NECESSARY. The contractor is responsible for determining the most cost effective method of mobilization. The work requires the contractor to clear and grub all areas in the required work area that have tall grasses, weeds, and underbrush. Trees and woody vegetation having stump diameters of eight inches or less shall be cut flush with the ground and all cut stumps shall be treated with a waterproof preservative to prolong rootball and stump decay. Existing trees larger than 8 inches are not included in this contract, and shall be left in place. An y areas that do not allow for the removal or grinding of the stumps, such as in rip-rap areas, are to be chemically treated to prevent new sprouts. The areas where the clearing and grubbed soil was removed must be backfilled with clean, compacted backfill , top soiled to a depth of four inches, seeded and mulched. Grass seed shall be close to 65% Kentucky Blue Grass, 20% Fine Fescue, 15% Perennial Rye Grass. The contractor is responsible for disposing of all debris generated by this project outside of the l imits of government land at contractors own cost. Burning will not be permitted on site. Site visits are required prior to bid submittal, and can be arranged by contacting Mr. Robert Stanick of the U.S. Army Corps of Engineers, Fox River Sub-Office at (9 20) 766-3531. After award of the contract, the contractor must also notify Mr. Robert Stanick 3 days prior to start of any work. Prior to performing any work on site, the contractor shall submit an accident prevention plan/activity hazard analysis that sp ecifically addresses the clearing and disposal of all debris. Contractor is also required to furnish a work plan that addresses clearing and grubbing. All work must comply with the requirements of the EM 385-1-1, U.S. Army Corps of Engineers Safety and Hea lth Requirements Manual, as well as all OSHA, State and local requirements. ITEM 0001: Menasha Dam, 1 Lump Sum Unit Price $__________. The required work area is approximately 0.40 acres located on the property adjacent to the government dam located in Me nasha, Wisconsin (See Figure 2). Item No. 0002 OPTION: Appleton Upper Dam, 1 Lump Sum Unit Price $__________. The required work area is an approximately 2,50 0 SF area adjacent to the left dam abutment (See Figure 3). Item No. 0003 OPTION: Appleton Lower Dam, 1 Lump Sum Unit Price $__________. The required work area is an approximately 10,000 SF area located along the right river bank between the navigation lock and the river (See Figure 4). Item No. 0004 OPTION: DePere Dam, 1 Lump Sum $________ _ . The required work area is an approximately 21,000 SF area between the navigation lock and the dam (See Figure 5). Item No. 0005 OPTION: Cedars Dam, 1 Lump Sum $____________. The required work area is an approximately 15,000 SF area between the lock and the dam (See Figure 6). Item No. 0006 OPTION: Kaukauna Dam, 1 Lump Sum $____________. The required work area is an approximately 4,000 SF area located alo ng the right river bank directly upstream of the right dam abutment. Care should be taken not to disturb the stone wall along this section of the river. Also, an approximately 15,000 SF area located directly below the right spillway of the dam is to be cle ared. In addition, the left canal wall is to be cleared of all brush in an area extending from the left dam abutment downriver to the Memorial Bridge and out 50 from the toe of the wall. (See Figure 7) Item No. 0007 OPTION: Little Kaukauna Dam, 1 Lump Sum $____________. The required work area is an approximately 20,000 SF area between the navigation lock and the dam (See Figure 9). Item No. 0008 OPTION: Little Chute Dam, 1 Lump Sum $___________. The required work area is an approximately 2,500 SF area located next to the right dam abutment (See Figure 10). Contractor is required to complete all work within 45 days of contract award. Type of contract will be firm fixed price. The provision 52.212-1, Instructions to Offerors Commercial Items is applicable. The provision 52.212-2, Evaluation Commercial Items is applicable. Offerors are to include a completed copy of provision FAR 52,212-3, Offeror Representations and Certifications, Commercial Items , and DFARS 252.212-7000 Offeror Representations and Certifications Commercial Items, with offers or a statement that these documents can be accessed at the ORCA website. Offerors must submit a completed offer schedule and acknowledgement of all amendment s issued. The successful offeror must be registered in the Central Contractor Registration at http://www.ccr.gov.
