Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, in conjunction with FAR 13.5, as applicable, and as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. The solicitation number is PC-080907 and is issued as a request for quotation (RFQ). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 05-17. The associated North American Industrial Classification System (NAICS) code for this procurement is 424210 with a small business size standard of 750 employees. This requirement is set-aside for small business vendors only. The Peace Corps requires the following items, Exact Match Only, to the following: CLIN 001, Gardasil Vaccine, 0.5ml single dose, Quantity = 3000. Specify delivery schedule. Offerors may quote all single vials, all syringes, or a combination thereof, so that the total quantity proposed equals 3,000 - 0.5ml single doses. SINGLE VIAL, NDC #00006-4045-00 PREFILLED SYRINGE, NDC #00006-4109-31 Delivery shall be made within 30 days or less after receipt of order. FOB Destination shall be Washington, DC 20526. Solicitation provisions at FAR 52.212-1, Instructions to Offerors-Commercial Item are hereby incorporated by reference. The Government will evaluate quotes in response to the synopsis/solicitation on the basis of price and delivery schedule which are equally important. A fixed price award will be made based upon the best value to the Government. The Government reserves the right to issue multiple awards for varying quantities if it results in the best value to the Government. The following clauses apply to this solicitation and the resultant contract. The selected Offeror must submit a completed copy of the provision at FAR 52.212-3, Offeror Representations and Certification-Commercial Items. FAR clause 52-212-4, Contract Terms and Conditions-Commercial, is hereby incorporated by reference. The following FAR clauses in paragraph (b) or FAR clause 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders-Commercial Items, will apply: 52.222-21, 52.222-26, 52.222-35, 52.222-36, 52.222-37, 52.225-13, 52.232-34.
