- This is a combined synopsis/solicitation for a commercial items contract, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes only solicitation, quotations are being requested and a written solicitation will not be issued. The solicitation number, VA-69D-07-RQ-0388 is issued as a request for quotation (RFQ). The North American Industry Classification System (NAICS) Code for this acquisition is 334510, Size: 500. This procurement is unrestricted. Quotations are due on or before 21 August 2007, 1:00 PM. Central Standard Time to be considered responsive. CLIN 0001: Force Triad Energy Platform, Catalog number: FORCETRIAD QTY: 01 U/I:EA $____________. CLIN 0002: Force Triad Mounting Cart, Catalog number: FT900 QTY: 01 U/I:EA $____________. CLIN 0003: Ligasure single footswitch, Catalog number: LS0300 QTY: 01 U/I: EA $____________. CLIN 0004: GEM Bipolar Footswitch, Catalog number: E6019 QTY: 01 U/I: EA $____________. CLIN 0005: Ligasure Single Pedal Footswitch, Catalog number: LF0500 QTY: 01 U/I: EA $___________. CLIN 0006: Standard Footswitch, Catalog number E6008 QTY: 01 U/I: EA $___________. Delivery, which shall occur within 30 calendar days of contract award, shall be FOB DESTINATION to WM S. Middleton VAMC, 2500 Overlook Terrace, Madison, WI 53705. The Contractor is responsible for all freight charges. The provision at 52.212-1, Instruction to Offeror--Commercial Items, applies to this solicitation. The provision at 52.212-3, Offeror Representations and Certification--Commercial Items, applies to this solicitation. The contractor shall return a completed copy of this provision with its quotation. A copy of the provision may be attained from http://www.arnet.gov/far/current/html/52_212_213.html or completed at https://orca.bpn.gov/. The resulting firm fixed price award will incorporate the requirements of the clause at FAR 52.212-4, Contract Terms and Conditions--Commercial Items. The clause at 52.212-5, Contract Terms and Conditions Required to Implement Statutes or Executive Orders--Commercial Items, applies to this acquisition. The Offeror shall also provide its Commercial and Government Entity code, DUNS number, and Tax Identification number. As of October 1, 2003, all Contractors, both large and small, are required to register in the Central Contractor Registration (CCR) database before an award. Contractors are responsible for the accuracy of information added to CCR. To register, go to http://www.ccr.gov/. The Federal Contractor Program requires that any contractor receiving a contract from the Federal Government in the amount of $25,000 or more, or any subcontractor receiving a contract in the amount of $25,000 or more from such a covered contractor must file a VETS-100 Report on an annual basis.
