Description
This proposed contract action is for supplies or services for which the Naval Medical Center San Diego intends to solicit with only one source under the authority of FAR 6.302. This notice of intent is not a request for competitive quotations. However, all quotes received within 7 calendar days after date of publication of this synopsis will be considered by the Government. A determination by the Government not to compete with this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement in the future. This is a combined synopsis/solicitation for commercial items, prepared in accordance with the format in FAR subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written sol icitation will not be issued. The solicitation number, N00259-07-T-0302, is issued as the request for quotation (RFQ) reference number. This acquisition is a 100% small business set-aside and order will be a firm fixed price order. The North American Industry Classification System (NAICS) Code for this acquisition is 325413: 500 Employees. All responsible small businesses may submit a quote that shall be considered by the agency. All quotations should be directed to Kohala Webster, Naval Medical Center San Diego, 34800 Bob Wilson Drive, San Diego, CA, 92134, or by e-mail to kohala.webster@med.navy.mil, on or before August 24, 2007, 2:30 p.m. Pacific Daylight Time to be considered responsive. No phone inquiries will be accepted. In accordance with FAR 52.212-2, Evaluation - Commercial Items (1999), award will be made based on technical ability, past performance, and price. These evaluation factors have equal levels of importance. For past performance evaluation, at lea st three (3) references of sources for whom you have provided the spec ified materials and/or service(s) MUST be provided. Contractor MUST be registered in the Central Contractor Registration (CCR) prior to award. The website address for CCR registration is www.ccr.gov. Contractor MUST complete FAR 52.212-3, Offeror Representations and Certifications ? Commercial Items (NOV 2006), when submitting a proposal. The website address is https://orca.bpn.gov/. The solicitation document and incorporated provisions and clauses are those in effect through FAC 2005-18. This requirement is for an annual standing order to include use of equipment for the base period of 01 October 2007 through 30 September 2008, with four (4) option years. CLIN 0001 (BASE YEAR): Period of performance 01 Oct 2007 through 30 Sep 2008. One (1) SPIFEY 3000 Analyzer and one (1) QUICKSCAN DENSITOMETER. Standing order for reagents listed in the following subCLINs to include use of one (1) SPIFEY 3000 Analyzer and one (1) QUICKSCAN DENSITOMETER. Use of equipment to include all co rrective and service maintenance, calibration, transportation and on-site training. Contractor shall retain ownership of the above mentioned equipment. Equipment to be provided to Government for use by Contractor for a period of one year. Contract to include the following reagents to be delivered quarterly or semi-annually in the quantity specificed per delivery as shown on the line item. CLIN 0002AA through CLIN 0002AG: Same item descriptions, delivery schedules and quantities are to be quoted as for CLINs 0001AA through 0001AG. CLIN 0003 (OPTION YEAR TWO): Period of performance 01 Oct 2009 through 30 Sep 2010, same as CLIN 0001. CLIN 0003AA through CLIN 0003AG: Same item descriptions, delivery schedules and quantities are to be quoted as for CLINs 0001AA through 0001AG. CLIN 0004 (OPTION YEAR THREE): Period of performance 01 Oct 2010 through 30 Sep 2011, same as CLIN 0001.
