- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in the Federal Acquisition Regulation (FAR) Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a quote is being requested and a written solicitation will not be issued. The reference number for this acquisition is F1J5AF7149A004. The solicitation document and incorporate provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-12 (4 Aug 2006) and Defense Federal Acquisition Regulation Supplement change notice 20060814. It is the Offeror?s responsibility to be familiar with applicable clauses and provisions. The North American Industry Classification System (NAICS) for this acquisition is 518210 with a business size standard: $23 Mil. The following acquisition will be a small business set aside, Numbered Note 1 applies. This commercial acquisition is for non-personal services for microfilm conversion to CD-ROM or DVD for the United States Air Force School of Aerospace Medicine (USAFSAM), Aeromedical Consultation Service (ACS), Clinical Records Section. Due to the nature of this acquisition the Government intends to make this a best value decision. The type of contract anticipated is firm-fixed price. The performance period for this task order will be 12 months from time contract is awarded. The following FAR and DFAR Clauses apply to this combined synopsis/solicitation and any subsequent award: 52.212-1 (Jan 2006), Instructions to Offerors Commercial; 52.212-2 (Jan 1999), Evaluation - Commercial Items (Evaluation will be based on technical capability and price); 52.212-3 (Jun 2006), Offeror Representations and Certifications - Commercial Items (Offerors must submit a completed copy with their offer); 52.212-4 (Sep 2005), Contract Terms and Conditions - Commercial Items; 52.212-5 (Aug 2006), Contract Terms and Conditions required to implement statutes of executive Orders - Commercial Items.
