- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; propo sals are being requested and a written solicitation will not be issued. Solicitation W81NTE71878507 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisi tion Circular 2005-18. This acquisition is solicited on an unrestricted basis under NAICS 339114 Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand name or Equal applies to this acquisition. The requirements of this solicitation are: Item CLIN 00 01 Dental Command Air Portable Field Sink (electrical or manual foot pump; electrical heater and pump; use standard military water containers for supply and disposal; stainless steel tray (6 1/2 x 10); adjustable water flow to a maximum of 2.2 liters per m inutes). CLIN 0002 Dental Command Air Portable Operator Stool (interchangeable stools for doctor and assistant; easy to fold up and carry); CLIN 0003 Dental Command Air Portable Light (bright halogen light on highly adjustable tripod stand; 10 degree beam ; Dual voltage 115/230v 50/60hz; weight 15lbs); CLIN 0004 Dental Command Air Portable Tray Stand (tripod stand with standard 13 x9 ? stainless instrument tray on top; removable tray for autoclaving; stand is height adjustable from 29 to 57); CLIN 0005 Dent al Command Air Chair (DOM steel frame with replaceable vinyl covering; function as standard sit-up chair; adjustable pillow headrest; dimensions 39 x 30 x16.5, 11.17 cu ft). Interested firms shall comply with FAR clause 52.211-6, Brand Name or Equal, if q uoting other than the brand name product. Clause 52.211-6 and other clauses cited throughout this solicitation can be viewed by accessing website: http://farsite.hill.af.mil. Quotes shall include all shipping and handling charges (FOB Destination) for d elivery to Brooke Army Medical Center, Building 3600, 3851 Roger Brooke Drive, Fort Sam Houston, TX 78234-6200. Vendor shall indicate on their price quote if shipping method will be other than FOB Destination. FAR provision 52.212-1 [Instructions to Offerors Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) pro gram. The CCR can be accessed via the internet at www.ccr.gov/ . Confirmation of CCR registration will be validated prior to awarding a contract. The Government will award a contract resulting from this combined synopsis/solicitation to the responsible firm whose quote is lowest priced technically acceptable. Technical acceptability is compliance with the requirements of FAR Clause 52.211-6, if quo ting on an equal product. Firms responding to this combined synopsis/solicitation shall include with their quote, a completed copy of the provisions at FAR 52.212-3 Offeror Representations and Certifications Commercial Items (Nov 2006).
