Description
This is a Sole Source Logical Follow-On requirement to be awarded to the encumbent (Note 22 Applies) 1.1 The mission of the Population Health Support Division (PHSD) of the Air Force Modernization Division (SGRKP) is to support Department of Defense (DoD) health professionals in optimizing the health and wellness of their populations through appropriate, effective and efficient healthcare practices and service delivery. 1.2 The development of the Integrated Delivery System Consultation Assessment Tool (IDS-CAT) and software was completed September 2004, was subsequently maintained to support the pilot test of the training program. This unit-level assessment of modifiable behavioral and personal risk factors has become a critical requirement during this period of heightened operational tempo and has high visibility throughout the Air Force. It was staffed and approved for AF-wide deployment by the AF Community Action Information Board (CAIB), September 2005 with a policy letter (CVA Memorandum) signed November 2005. Full deployment of the training program with subsequent availability for field application began December 2005 and remains ongoing. 1.3 The contractor shall provide consultation services and direct delivery of the IDS-CAT training. The contractor will assist with the development of the training materials/activities; provide consultation to the field either by phone or by direct visit; and serve as the lead trainer in providing the direct delivery of the IDS-CAT training. The consultation services and direct delivery of the IDS-CAT training will occur in San Antonio, TX, and in the field. The contractor will be required to travel (approximately 12 trips in FY08; 10 in FY09; and 8 in FY10, primarily CONUS. Hours of performance shall be determined and jointly negotiated and agreed upon by the contracting officer technical representative (COTR) and the contractor, dependent on need. Hours will be flexible. The nature of the work will require effort varying from intense effort concentrated over a period of days to minimal effort. Awarding and continuation of this contract is contingent on available funding. 2.0 DESCRIPTION OF SERVICES. 2.1 SUMMARY OF SERVICES REQUIRED. This is a Sole Source logical follow-on contract for Nelson Consulting, and will establish a base period plus two option years beginning FY08. This position is required to provide a vital service for the sustainment of this AF-wide Suicide Prevention requirement. It is a time and materials contract.
