- The 55th Civil Support Team / Weapons of Mass Destruction, 612 Militia Drive, St. Paul, MN has a requirement for Medical Surveillance Testing for approximately 15 individuals. This requirement is being restricted to within a 60 mile radius of 612 Mi litia Drive. Testing shall be conducted at the Contractors location. All testing shall be in accordance with Army Regulation 40-501, Chapter 3. This requirement shall consist of physical examinations (head-to-toe), HIV laboratory tests, hearing / audiogr am examinations, vision examinations, urinalysis dips laboratory tests for albumin/sugar and specific gravity, drug screens, prostrate examinations, EKG testing, rectal exams, urine laboratory tests, intraocular pressure examinations, PSA laboratory tests, Chem-24 and/or equivalent laboratory tests to include fasting lipid profile including total cholesterol, LDL, HDL and triglycerides; fasting blood sugar, CBC laboratory tests to include HGB or HCT values, serum lead laboratory tests, serum mercury laborat ory tests, spirometry examinations, random urine heavy metal laboratory tests, serum cholinesterase laboratory tests drawn as 2 (two) values on 7 (seven) personnel as identified, Hep B Saby tests, Hep C sapy tests, CPK tests, chest x-rays and optional item s as needed such as female pelvic exams, PAP smears and Mammography. The estimated solicitation issue date is o/a 5 September 2007. The estimated proposal due date is o/a 14 September 2007. A firm fixed-price Indefinite Delivery/Indefinite Quantity Cont ract is anticipated with a Base Year and Three Option Years. The North American Industrial Classification System code for this acquisition is 621399 and the size standard is $6.5 mil. This requirement is being reserved with preference for Small Business Set-Aside under a new and innovative concept. This concept is referred to as the Cascading Set-Aside for Small Business Awards . Large business concerns are put on notice of the preference for award to small business concerns. If two or more offers fro m qualified small business concerns are received, this solicitation shall be converted to a 100% Small Business Set-Aside, as defined by FAR Clause 52.219-6. In such event, FAR Clause 52.219-6, Notice of Total Small Business Set-Aside shall be considered incorporated into the solicitation by reference. The solicitation package will be issued via the web site only. All amendments, if issued, will also be posted on the web.
