Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes amendment to the solicitation; pr oposals are being requested and a written solicitation will not be issued. Solicitation number: W9114F-07-T-0238 issued as a request for quotation (RFQ). A firm-fixed price type contract will result from this solicitation. Solicitation, amendments and other miscellaneous documents will be posted to FedBizOpps (http://www.eps.gov). It is the responsibility of each offeror to review the web page for notice of amendments, updates or changes to current information. This acquisition will be conducted usin g Full and Open competition procedures. The Europe Regional Contracting Office has a requirement to purchase the following items: Description of requirements for the items to be acquired: CLIN 0001 Automatic Refractor, NIDEK RT 5100 Quantity 2 eac h, Electrical Requirements 220V/50Hz; 2 copies each of Operator and Maintenance/Service Manuals in English Language; CLIN 0002 Chart Projector, NIDEK CP 690, Quantity 2 each, Electrical Requirements 220V/50Hz; CLIN 0003 Instruments Table System, MOD Numb er 10250, Quantity 2 each, Electrical Requirements 220V/50Hz; CLIN 0004 Armchair, Model number P100, Quantity 2 each, Electrical Requirements 220V/50Hz; CLIN 0005 Refractors Arm, Quantity 3 each, Electrical Requirements 220V/50Hz; CLIN 0006 Slit Lamp for table system, Model Number SL 990, Quantity 2 each, Electrical Requirements 220V/50Hz; CLIN 0007 Tonometer, Quantity 2 each, Electrical Requirements 220V/50Hz, CLIN 0008 Installation, final check and training. CLIN 0009 Operating and Maintenance manuals must be provided in English language, Quantity 2 Sets. Warranty statement will be provided in English. The place(s) of delivery and acceptance will be Landstuhl Regional Medical Center, Property Management Branch, Bldg 3739, Landstuhl, 66849, Germany. Th e contractor will provide the earliest delivery date. All offerors must quote FOB Destination, Landstuhl Regional Medical Center, Landstuhl, Germany.
