Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. Simplified acquisition procedures apply and the VA will conduct the procurement according to a combination of FAR Parts 12 and 13. This announcement constitutes the only solicitation issued. Quotations are being requested and a separate written solicitation will not be issued. Request For Quotation (RFQ) VA-69D-07-RQ-0473 entitled Slit Lamp is issued as an unrestricted procurement. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-19. No telephone quotes will be accepted. NAICS code 423450 applies with the small business size standard of 500 employees. Offeror's quotes are hereby being requested and shall be submitted on offeror's letterhead or bid form and shall contain the name, title, and signature of person authorized to submit quotes on behalf of the contractor. Contractor shall provide the following Brand Name or Equal items: Item 1: Ziess SL130 Slitlamp with Pedestal Table, Lumenis product number GA-0034830 1 each. Item 2: Ziess SL130 Observation Tube, Lumenis product number 3302-0071 1 each. Item 3: Ziess SL130 Beam Splitter, Lumenis product number 3302-0075 1 each. Item 4: Ziess Laserlink Z, SL130, Omni, Lumenis product number 0631-675-01 1 each. All items must be compatible with a 2000 Coherent Novus Omni Laser. Contractor will provide two (2) operators manuals and one (1) service manual for each piece of equipment. All items provided will comply with NFPA 99 code. Brand name listed products have hospital grade 3 wire power cords with 120VAC, 60 Hertz, Single Phase power requirements. Delivery is to be FOB Destination to the William S. Middleton Memorial Veterans Hospital, 2500 Overlook Terrace, Madison, Wisconsin, 53705. If offering other than Brand Name offeror must complete the following: Bidding On Manufacturer Name __________, Brand _________, Number ___________. The Government will award a contract resulting from this solicitation to the responsible offeror whose offer is the most advantageous to the government, price and other evaluation factors considered. Evaluation factors used to evaluate offers are Technically acceptable, Price, Delivery, Veteran owned small business, and Service-Disabled Veteran owned small business. Contractor must provide descriptive literature if offering other than Brand Name to demonstrate that the offeror item is an equal product. Offeror must register in Central Contractor Registration Database and provide DUNS number for verification.
