- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented, with the additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. For solicitation number VA256-07-RQ-0241, award will be made using the simplified acquisition procedures part 13.5. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-19. The North American Industry Classification System # 334517 and the small business are 500 employees. The acquisition is for the purchase of 1 each of Medrad product No. 3012011 Spectris Solaris EP MR Injector, 1 each Medrad product No. 78101-00-BM-01 Product inform pack, 2 each Medrad Product No. 535-0243-012 North American style power cord, 1 ea oMedrad product No. 3012080 Spectris Solaris EP, CBC, 1 each Medrad product No. 203943 Addedum Spectris Solaris EP, OP Manual, 1 each Medrad Product No. 203089 Spectris Solaris EP iCBC manual paper and 1 each Medrad product No. SSI 300 Installation Spectris Solaris MR Injector (brand name), equal or better. If or equal, supply adequate information to determine whether the equal can be used. The quoter shall provide pricing and warranty information on the above listed equipment/supplies. Delivery is to be made to Chief, A&MMS, VA Medical Center, 2495 Shreveport Highway, Pineville, LA 71360. Delivery of equipment is desired between September 25, 2007 and October 19, 2007. Inspection and acceptance is at the VA Medical Center Alexandria, LA. Quote shall specify FOB, VA Medical Center Alexandria, LA (Destination). The provisions at FAR 52.212 1, Instructions of Offeror Commercial Items (Sept 2006) applies to this acquisition. There is an addenda to these provisions. Additional contract requirements: The vendor responding to this acquisition shall provide this office with the following information: a cost per item on company letterhead (equipment and consumables supplies), stating unit price, warranty, FOB, prompt payment terms, correct remittance address, and a completed copy of FAR 52.212 3 (Aug 2007), Offeror Representations and Certifications Commercial Items. If offering an equal product, the vendor must submit documentation of the product with the quote. A company not in possession of FAR 52.212 3 dated Jun 2006 it has been provided as an attachment. Quotes must be in writing and accompanied by a completed Representations and Certifications (FAR 52.212-3) dated Aug 2007 or they will be rejected. Facsimile quotes will be accepted; oral quotes will not be accepted. Award will be made to the offeror whose quote conforms to the requirements of this solicitation, best meets the minimum needs of the Government and has the lowest price, all factors being considered. Award made be made without discussions.
