Description
This is a combined/synopsis solicitation for commercial items prepared in accordance with the format in Subpart 12.6 as supplemented with additional information included in the notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation reference number is F3Z4537178A013; this solicitation is a request for quotation. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18 and Defense DCN 20070531. The associated Standard Classification (SIC) is 3669. The North American Industry Classification System is 621512 and the business size standard is $12,500,000.00.. The Federal Supply Class (FSC) is Q532. This solicitation is unrestricted. The government intends to solicit and award this purchase order for the following item: SERVICES, NONPERSONAL: IAW with the Performance Work Statement, contractor will furnish labor, tools, parts, materials, transportation, equipment, software, training and lectures required to include the mobile Positron Emission Tomography/Computed Tomography imaging unit, tractor, trailer, computer hardware, and software and any other equipment necessary to perform PET/CT exams at David Grant Medical Center (DGMC) AFB with capability to save images to a disk and transmit images to DGMC electronically via a CAT 5 line for use by DGMC providers for interpretation of images. The contractor will make the equipment available to DGMC?s patients on 30 Sep 2007 and beginning on 1 Oct 2007 through 15 Sep 2008, two visits per month to include two option years.
