Description

This solicitation is UNRESTRICTED for commercial items prepared in accordance with the subparts in Federal Acquisition Regulation 12, and Veterans Administration Acquisition Regulations as supplemented with additional information included in this notice. Solicitation number RFQ 673-127-07 applies and is issued as a request for quotations. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-19. The North American Industry Classification Systems (NAICS) code is 811219, size standard $6.5 million. The James A. Haley Veterans Hospital (JAHVH) is soliciting quotations from qualified, responsible firms who have technical expertise and necessary resources to provide FULL MAINTENANCE AND REPAIR SERVICES (Includes telephone & On-Site Service) FOR BLOOD GAS ANALYZER EQUIPMENT located at the ORLANDO VETERANS AFFAIRS MEDICAL CENTER (OVAMC), ORLANDO, FLORIDA, 5201 Raymond Street, Orlando, Florida 32803. Equipment to be serviced/maintained includes: Blood Gas Analyzer, Mfg: Ciba-Corning, Model# 845, Ser# 07282. Period of contract shall be from October 1, 2007 through September 30, 2008 with four optional years. All quotations are to be submitted by hard copy to the designated office by COB September 17, 2007. ELECTRONIC VERSIONS OF QUOTATIONS ARE NOT ACCEPTABLE. Electronic version and hard copy of solicitation will be available September 6, 2007. Electronic solicitation may be obtained FREE at the following URL: http://www.va.gov/oa&mm/busopp/sols.htm. Any amendments or additional information issued to this solicitation will also be available electronically at the URL address. Offerors are responsible for obtaining any and all amendment or additional information. Requests for hard copy solicitation must be in writing. No telephone requests will be accepted.
