Description
Statement of Work for Neuropsychiatric and Physiatry Services 1. To provide onsite Physiatry Assessment Services to include but not limited to: Assessment Services would include a Physiatrist on site that would be available to evaluate identified patients who have sustained injuries related to military services. The compliment of services required is as follows: Rehabilitation Physiatrist assessment on location 1x per month Physiatrist Assessment to include: Present Illness Past Medical History Medication Allergies Social Premorbid function Review of systems Physical exam to include Comprehensive spasticity/tone assessment Neurological Functioning Pain Assessment Functional Assessment 2. To provide onsite Neurorehabilitation assessment to include but not limited to: Rehabilitation Neurology assessment on location 1x per quarter, or as needed Neurology assessment to include: History of Present Illness Past Medical History Medication Allergies Personal/Social/Occupational History Focused System Review Physical Exam with special attention to Neurobehavioral Status 3. To provide an onsite battery of Neuropsychiatric testing to include but not limited to: Neuropsychological Assessment would be prescribed on an as needed basis following the rehabilitation neurology assessment. Assessments are completed when it is determined that only through the extensive battery can an adequate treatment plan be established. Inclusion Elements Review of records Clinical interview Behavioral observation Administration of standardized measures General Observations Appearance Motor Abilities/stamina Affect Communication Thought Patterns Attitude toward examiner/self Reactions to test Work habits and approach Standardized Measures Cognition/Academic Neurobehavioral Emotional Functioning Tests and Area of Functioning: (6 - 8 hours of testing) " Academic Skills: arithmetic calculation/reasoning, reading comprehension, word reading, spelling, writing " Attention/Concentration " Nonverbal and Verbal Memory: immediate, delayed, learning, incidental learning " Visual Based Skilled: perception, reasoning, motor, construction " Language Skills: vocabulary, oral fluency, naming, expressive language, receptive language " Reasoning and Judgment: verbal reasoning, nonverbal reasoning, judgment and safety, self-awareness 4. To provide onsite consultative services to the Polytrauma/TBI Interdisciplinary Team, Primary Care, and Specialty Care providers. The Physiatrist and Rehabilitation Neurologist will be part of an interdisciplinary team, assisting with the rehab needs of our Polytrauma and TBI population throughout the continuum of care. This will include recommendations for treatment and follow-up, as well as patient/family education re: treatment options . 5. To provide offsite comprehensive pain assessment to include but not limited to: " Evaluation of chronic pain due to spinal pathologies (CT-Scan and MRI results as needed) " Assessment/ treatment recommendations for neuropathic pain " Assessment/ treatment of pain syndromes due to immobility/tone changes. " Review of past and present treatments (medications, injections, surgeries, therapies) " Review of medical history, allergies, and current medications 6. To provide comprehensive spasticity/tone management and pain treatment recommendations to include but not limited to: " Botox injections " Baclofen injections " Phenol injections " Intrathecal pump management " Spinal intervention therapy (examples , but not limited to epidural steroid injections, facet injections, nerve blocks, radio-frequency lesioning, kyphoplasty,) " Medication management recommendations. 7. To provide offsite comprehensive inpatient and outpatient interdisciplinary cognitive/behavioral and/or physical rehabilitation services necessary for the care of TBI patients, when indicated. This will include but not be limited to: " Physical Therapy " Occupational Therapy " Recreational Therapy " Speech Therapy " Physiatry " Rehabilitation Neurology " Pain/tone specialist " Rehabilitation Nursing " Mental Health
