- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The Employee Education Service, Veterans Affairs Medical Center, Brecksville, Ohio propose to enter into a non-competitive commercial supply/service contract with the Utah Scientific Incorporation, 4750 Wiley Post Way, Salt Lake City Utah 84116 to provide media routing system that is compatible with the existing UTAH 200 and 400 routing system with the following specifications: 1. Video: 64 x 64 matrix frame configured with 48 Serial Digital Interface (SDI) video inputs by 48 SDI video output router with plug in expansion a full 64 inputs by 64 outputs. Quantity 1 each. 2. Audio: 64 input Analog balance line level audio by 64 analog balanced line level outputs dual channel audio., Quantity 1 each. 3. Control: 64 port bi-directional RS-422 video machine control for device control assignable Master or Slave per cross-point. 4. Master Operator control panels: U-Net based control system 16 level control with direct take functions single or multi destination with 20 button key pad and Editor Control Panel 4 direct take button with 8 level control and 20 button keypad. 5. Power: Redundant power supply system for each chassis. Manufacturer Product Numbers: 1. 40064SD48x48; quantity 1 each; 2. 30064AA64x64, quantity 1 each; 3. PS3AC4WSYSR, quantity 1 each; 4. UTAH400DATA, quantity 1 each; 5. RJ459D - RS-422 RJ45 to 9Pin D connector (female) quantity 64 each; 6. UCP-MX, assignable source control panel, quantity 2 each; 7. UCP2 Source control panel, quantity 8 each; UTAH400RS422 UTAH-400 spare VTR machine control (64) cross point card, quantity 1 each. Offeror(s) submitting proposal(s) for products that are considered equal shall demonstrate that the product(s) being offered are compatible with the existing equipment. The following provisions apply to this acquisition: 852.270-4 Commercial Advertising; 52.212-1 Instructions to Offerors-Commercial Items; 52.212-2 Evaluation-Commercial Items, paragraph (a) is filled in as follows: The Government will award a contract resulting from this solicitation to the responsible offeror whose offer conforming to the solicitation will be most advantageous to the Government price and other factors considered. The following factors will be used to evaluate offers: Technical, past performance and cost. The technical criteria and past performance are approximately equal in weight, and when combined are approximately equal to price; 52.212-3 Offer Representations and Certifications-Commercial Items; 52.212-5 Contract Terms and conditions to Implement Statutes of Executive Orders-Commercial Items, including clauses incorporated by reference under Section (b) 15 through 21, and 33. Offerors responding to this announcement shall provide the information required in provisions 52.212-1 and a completed copy of 52.212-3.
