- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested. Solicitation number is VA-250-07-RQ-0166 issued as an RFQ (Request for Quote). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acuisition Circulars (FAC) 2005-18. This requirement is 100% service disabled veteran small business set aside for commercial items under the North Atlantic Industrial Classification System (NAICS) 339115 Ophthalmic Goods Manufacturing, Size standard 500 employees. Requirements: The Louis Stokes Cleveland Department of Veteran Affairs Medical Center (LSCDVAMC), 10000 Brecksville Road, Brecksville, Ohio 44141, (hereafter known as the LSCDVAMC), is seeking a system that diagnoses eye disorders affecting vision. Line Items; (1) LKC UTAS Visual Diagnostic Test System with MFERG/ MFVEP: UTAS Desktop=MFERG/MFVEP Quantity of 1 Each (2)Desktop Computer, Rewriteable cd-rom drive,flat LCD screen,HP deskjet color printer or compatible mouse and keyboard,UBA 4204 Signal Acqusistion SYStem with 4 Recording Channels, Trigger input/output(Negative Going TTL), MGIT-100 Medical Grade Isolation Transformer cables, Quantity of 1 Each. (3) Sunburst Fully Automated Color LED Ganzfeld, Quantity of 1 Each. (4) Windows XP operating system, Quantity of 1 Each. (5) EM Windows v. 7.0 or greater SunBurst produces all ISCEV standard stimuli, PLUs double flash ERG, on/off response ERG, On/Off Response ERG, S-Cone ERG, Chromatic backgrounds and stimuli(photopic negative response), scotopic threshold response ERG, Infared flash for retinal implant testing user-defined protocol capability, data storage and retrieval, flexible printing options, export capability, Quantity of 1 each. (6) EOG Protocol, Quantity 1 each, (7) Multi-Focal ERG and Multi-Focal VEP software for Windows, Quantity 1 each, (8) Pattern VEP, Pattern ERG Protocols, Quantity 1 each, (9) 25 ERG-Jet, 3 VEP Scalp, 1 Ground Ref, Quantity 1 each, (10) EOG-Sup Disc, 100 EOG Electrode Washers, (11) 1 tube Electrode gel, 1 tube electrode cream, 1 box skin cleanser, 30 Disposable Ground Electrodes, Quantity 1 each, (12) 1 Bi Polar contact Lens Electrode, 2 ear clip, Quantity 1 each, (13) UTAS Electronic and hard copy and electronic User manual, Quantity 1 each, (14) Multi-Focal Hard copy and electronic user's manuals, Quantity 1 each, (15) Compact workstation, Quantity 1 each, (16) Instrument Table, quantity 1 each, (17) 12 months full parts and labor warranty, Quantity 1 each, (18) 2 days onsite installation and training, Quantity 1 each. Equal items require specification sheets, manufacturer, and part numbers. FOB destination is preferred. Shipping of over $250 will be provided by the Government through a third party carrier. PROVISIONS AND CLAUSES: Offerors must comply with all instructions contained in provision 52.212-1, Instructions to Offerors-Commercials, applies to this acquisition. Offerors shall thoroughly review the nineteen requirements listed above and provide individual pricing on each line item. Each line item must have a description and pricing information. Quotes must contain a proposed installation and educational training timeline. The following addendum to the provisions, delete paragraphs (h) and (i) of the standard provisions. In accordance with Federal Acquisitions Regulations (FAR 52.212-2 Evaluation-Commercial Items) all offers will be considered Best Values in descending order of importance. Technical elements are significantly greater than the price element. (1) Technical: Acknowledgement of ability to acquire product; a copy of specifications for other than brand name product. (2) Past Performance: Please include three references of customers who have purchased a similar system. (3) Price: Quote for each line item. (4) Warranty: Indicate Terms in statement of work; (5) Installation: 30 Days FOB: The Government reserves the right to award without discussions, vendors, shall submit the most favorable terms. Small businesses, especially veteran-owned and service-disabled veteran owned small businesses, are encouraged to contract their nearest Procurement Technical Assistance Center (PTAC) if assistance is needed in preparing their proposal.
