- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. This solicitation, F2E3CS7222A001, is being issued as a Request for Proposal (RFP). This announcement will be issued as a combined synopsis/solicitation. The anticipated award will be a Firm Fixed Price Contract, the award will be made based on the lowest price technically acceptable to the Government. This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-18. This acquisition is to be 100% set aside and awarded to Small Business. The NAICS code is 238210. The Small Business size standard for this code is $13M. The following commercial services are requested in this solicitation; CLIN 0001: Contractor will provide Category 5E (350Mhz & 10 Pin RJ-45) cabling system to support the LAN upgrade for buildings 49, 54, 55, 58, 149, 195, 280, 281, 282, 374, 1099 (total of 752 drops), Patch Panels, Face Places, and RJ 45 Jacks. Both single and dual cable drops in each building, based on the initial walk through. CLIN 0002: Installation to include New Mexico Gross Receits Tax IAW Statement of Work. Each building will require a final walk through by both the Contractor and a USAF representative. Contractor will provide a 50-year limited warranty on all cable drops installed by Contractor. Statement of work and drawings can be obtained from SSgt Armando Rodriguez. Period of Performance shall be 13 weeks ARO, FOB: Destination of services is Holloman AFB, NM, 88330. END OF CLAUSE FAR 52.212-5 -- Contract Terms and Conditions Required to Implement Statutes or Executive Orders -- Commercial Items (Deviation) (a) Comptroller General Examination of Record. The Contractor agrees to comply with the provisions of this paragraph (a) if the contract was awarded using other than sealed bid, is in excess of the simplified acquisition threshold, and does not contain the clause at 52.215-2, Audit and Records-Negotiation. (1) The Comptroller General of the United States, or an authorized representative of the Comptroller General, shall have access to the right to examine any of the Contractor's directly pertinent records involving transactions related to this contract. (2) The Contractor shall make available at its offices at all reasonable times, the records, materials, and other evidence for examination, audit, or reproduction, until 3 years after final payment under this contract or for any shorter period specified in FAR Subpart 4.7, Contractor Records Retention, of the other clauses of this contract. If this contract is completely or partially terminated, the records relating to the work terminated shall be made available for 3 years after any resulting final termination settlement. Records relating to appeals under the disputes clause or to litigation or the settlement of claims arising under or relating to this contract shall be made available until such appeals, litigation, or claims are finally resolved. (3) As used in this clause, records include books, documents, accounting procedures and practices, and other data, regardless of form. This does not require the Contractor to create or maintain any record that the Contractor does not maintain in the ordinary course of business or pursuant to a provision of law. (End of Clause) HAFB 009 - WIDE AREA WORKFLOW STATEMENT (AUG 2006) In order to receive awards from the 49th Contracting Squadron at Holloman AFB, New Mexico, you must now use an E-Invoicing system called Wide Area Work Flow (WAWF). WAWF is a web-based tool for the processing of invoices/receiving reports. This will bring about major changes in the amount of time it takes to process these documents making payment much faster. Their number is 1-866-618-5988. HAFB 012 - HAFB CELL PHONE POLICY (31 Aug 2006) MANDATORY: Contractor personnel shall adhere to the base cell phone policy. Vehicles operated on a Department of Defense (DoD) installation, such as Holloman AFB, NM, will not use cell phones unless the vehicle is safely parked or unless they are using a hands-free device. The wearing of any other portable headphones, earphones, or other listening devices (except for hands-free cellular phones) while operating a motor vehicle is prohibited. Penalty: $25 fine (+$25 fee). END OF STATEMENT. HAFB 013 Effective date: 11 Jun 07 Insurance Coverage on Government Installation In accordance with 28.306, Insurance under fixed-priced contracts, paragraph (b)(1), contractor shall carry the minimum coverage of insurance as outlined in FAR 28.307-2, Liability; (a) Workers' Compensation and employer's liability; (b) General liability; (c) Automobile liability; and if applicable (d) Aircraft public and passenger liability or (e) Vessel liability.
