Description
This is a Combined Synopsis/Solicitation for the purchase of commercial service, prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. Solicitation number W911S8-07-T-0033 is applicable and is issued as a request for quotations. The solicitation and incorporated provisions and clauses are those in e ffect through Federal Acquisition Circular 2005-20 and DCN 20070906. This requirement is RESTRICTED to small business with a NAICS Code of 561730-Landscaping and a corresponding size standard $6.5 Million. The description of the commercial item service i s for Vegetation Removal at Gray Army Airfield Fort Lewis, and reads as follows: CLIN 0001, Statement of work (SOW): refer to the attachment on full SOW. Remove small trees, blackberry bushes. Scotch broom and various other shrubs and Gray Army Air Field at Fort Lewis Washington. CLIN 0002, Contractor Manpower Report: If there is not going to be a charge associated with completing the Services Contract Manpower Reporting requirement, please enter $0.00 charges to Government. The Office of the Assistant Secretary of the Army (Manpower & Reserve Affairs) operates and maintains a secure Army data collection site where the contractor will report all contractor manpower (including sub-contractor manpower) required for performance of this contract. The contrac tor is required to completely fill in all information in the format using the following address: https://contractormanpower.army.pentagon.mil. The required information includes: (1) Contracting Office, Contracting Officer, Contracting Officer's Technical R epresentative; (2) Contract number, including task and delivery order number; (3) Beginning and ending dates covered by the reporting period; (4) Contractor name, address, phone number, e-mail address, identity of contractor employee entering data; (5) Est imated direct labor hours (including sub-contractors); (6) Estimated direct labor dollars paid this reporting period (including sub-contractors); (7) Total payments (including sub-contractors); (8) Predominant Federal Service Code (FSC) reflecting services provided by contractor (and separate predominant FSC for each sub-contractor if different); (9) Estimated data collection cost; (10) Organizational title associated with the Unit Identification Code (UIC) for the Army Requiring Activity (the Army Requirin g Activity is responsible for providing the contractor with its UIC for the purposes of reporting this information); (11) Locations where the contractor and sub-contractors perform the work (specified by zip code in the United States and nearest city, coun try, when in an overseas location, using standardized. Delivery time from statement of work (SOW) the contractor shall be required to (a) commence work under this order within 3 calendar days after the order is issued, (b) prosecute the work diligently, an d (c) complete the entire work ready for use not later than 30 calendar days after issuance of notice to proceed. The time stated for completion shall include final cleanup of the premises. Acceptance will be at the destination. The selection resulting f rom this request for quotations will be made on the basis of the lowest priced, technically acceptable quote from a responsible quoter with acceptable or neutral past performance. Once the Contracting Officer has decided to whom an order will be issued, a bilateral order will be issued using a SF 1449. A purchase order will be in existence once the Contracting Officer has signed the order. Provision 52.212-1, Instructions to Offerors-Commercial Items, applies to this acquisition. An addendum to this pro vision is applicable, in which paragraphs (g) and (h) of FAR 52.212-1 are deleted. Wherever the words offer, proposal, offerors, or similar terms are used in this solicitation or any provision, they shall be read to mean quote, quotation, quoter, vendor or similar corresponding term to reflect that this solicitation is a Request for Quotations, not a Request for Proposals or an Invitation for Bi ds. Local clause 52.111-4131 states in the preamble: Procedures for commercial v ehicle access to Fort Lewis are subject to change without prior notice. Local clause 52.111-4132 states in paragraph f: The final invoice will not be considered proper for purposes of the Prompt Payment Act (FAR 52.232-25 or FAR 52.212-4(i)) until all i dentification badges have been accounted for. Quoter are required to submit completed copies of the provisions at FAR 52.212-3 and its ALT I, Offeror Representations and Certifications-Commercial Items, and DFARS 252.212-7000, Offeror Representations and Certifications - Commercial Items with their quotes. FAR clause 52.212-4, Contract Terms and Conditions-Commercial Items, applies to this acquisition and does not include any addenda. No numbered notes apply to this combined synopsis /solicitation. STATEMENT OF WORK AS FOLLOWS (SOW): STATEMENT OF WORK Vegetation Removal IJO # DTD000227J PKG #443 Date: August 22, 2007 1. DESCRIPTION OF WORK: Provide all plant, labor, tools, equipment, and materials (except as indicated otherwise in this Statement of Work (SOW)) and perform all work in strict accordance with National and Local Building, Plumbing, Mechanical, and Elect rical Codes a nd this SOW for Vegetation Removal. Vegetation consists of small trees, blackberry, scotch broom, various other shrubs. 1.1 The Principal Items to be Accomplished are: 1.1.1 Mow, brush cut and grub approximately 10 acres on Grays Army Airfield as indicated in figure 1 and figure 2. Cut all small trees less than 6 inch diameter in areas indicated on figure 1 and figure 2 to 3 inches of grade. 1.1.2 Access roads to be cleared two feet on present sides were possible. Mowed areas height limit is eight inched. Excess vegetation will be stock piled in designated area. 1.2 Optional Items None. 1.3 Work Not Included: Grass cutting. 2. PROJECT PHASING: The contractor shall notify the Government at least three working days prior to starting the on-site phase of the project. The following phasing related issues shall affect the work. 3.1 Interface with Others: Government use of these facilities is anticipated while the work under this contract is being performed. The work shall be planned and accomplished so that there will be a minimum of interference and inconvenience to occupant s in the building and vicinity. Any blockage of building exits or driveways must be coordinated in advance. 3.1.1 For work on the airfield area coordinate work with base operations 967-6628 /5998. After contract award the airfield commander Robert Rodriguez will authorize the contractor access to the airfield after the contractor has passed an airfield driver s afety test. 3.2 Project Restrictions: No work in airfield area during flight operations. 3.3 Utility Outages: Work should be scheduled such that utility outages are minimized. The following specific restrictions apply to this project. None 4. WORK HOURS: Normal work hours are from 0730 to 1600, Monday through Friday except Federal Holidays. Contractor shall not be permitted to work other than normal work hours unless authorized by the Project Manager. 5. AVAILABILITY OF UTILITIES: Subject to available supply, reasonable amounts of potable water and electricity will be furnished by the Government, without charge, to the contractor from existing outlets and supplies. Contractor shall reasonably conser ve these resources. Contractor shall provide, at its expense, sanitary accommodations for the use of employees and shall comply with the requirements and regulations of the State Health Department, County Sanitarian, or other authorities having jurisdicti on. 6. SAFETY: All work and materials used shall be in compliance with the provisions of the U.S. Department of Labor, Occupational Safety and Health Act, pertinent national fire prevention codes, the latest National Electrical Code and the U.S. Army Corps of Engineers EM 385-1 Safety and Health Manual. 7. CONTRACT PERIOD: The contractor shall be required to (a) commence work under this order within 3 calendar days after the order is issued, (b) prosecute the work diligently, and (c) complete the entire work ready for use not later than 30 calendar da ys after issuance of notice to proceed. The time stated for completion shall include final cleanup of the premises. 8. SUBMITTALS: The Contractor shall provide the specific submittal requirements indicated below. Submittals shall be approved by the Government before the Contractor begins work related to the submittals. a. Project Schedule b. MSDS for dust palliative material 9. ISO 14001 Implementation: PW is responsible for day to day management of the Fort Lewis environmental program. The PW Environmental Management System (EMS) outlines procedures and provides guidance that addresses the operation, maintenance, support, an d repair of facilities an d infrastructure at Fort Lewis. The PW EMS conforms to the criteria defined in the international standard, ISO 14001:2004 Environmental management systems  Requirements with guidance for use. The Contractor shall be familiar with PWs environmental policy and shall insure that this information is considered and incorporated into this project. The policy can be found at www.lewis.army.mil/publicworks, click on Environmental, then click on PW En vironmental Policy. Additional information on the PW EMS can be obtained by contacting the PW EMS representative. 10. INSPECTION AND ACCEPTANCE: 10.1. The Contractor and a Fort Lewis Public Works Representative will jointly conduct a pre-final inspection prior to the Contractor requesting a final inspection. Any discrepancies noted shall be corrected prior to the final inspection. 10.2 The Contractor shall request final inspection at least three working days prior to the desired date. If all deficiencies from the pre-final inspection have been corrected the Government will accept the work. If deficiencies have not been corrected or further deficiencies are identified, a timeframe will be agreed upon to correct all deficiencies. 10.3 The Contractor may bill for work completed subsequent to correction of all deficiencies and official acceptance by the Government. Only the PW Service Representative is authorized to make acceptance for the Government and acceptance will be annotate d on this Statement of Work. 11. ASBESTOS SURVEY: If needed, inspections shall be accomplished in accordance with the Asbestos Hazard Emergency Response Act (AHERA), Federal and Local regulations. A Certified AHERA Asbestos Building Inspector shall perform all inspections. The in spection documentation shall comply with AHERA, Federal and Local regulations and shall include assessment of each asbestos containing material (ACM), its location, quantity, type, friability and percentage of asbestos. 12. LEAD PAINT SURVEY: Contractor shall perform a complete lead paint inspection of all areas affected by the work described in this SOW in accordance with all Federal and Local regulations. A Certified Lead Paint Inspector shall perform the inspection.
