- a. Contractor shall provide qualified Clinical Perfusionist or Autotransfusionist Services to staff Omaha VAMC needs on a 24 hour a day/ 7 days a week basis for the period October 1, 2007 through September 30, 2008 and subsequent option periods. b. Contractor must be present for emergent cases no later than two hours after notification. Emergency notification contact information must be provided to the VAMC Operating Room Nurse Manager. c. Contractor must be present for elective cases no later than twenty-four hours after notification. d. Contractor must possess, demonstrate and maintain the technical capability to perform these services. This must include the ability to work independently, display an intimate knowledge/skill of required equipment and demonstrate ability to appropriately manage supply inventory for use with these services. e. The equipment and supplies necessary to perform these procedures will be VA-procured and VA-maintained. f. The Omaha VAMC provides inpatient and outpatient care to the veteran population in the Omaha catchments area. g. Contractor shall perform all services under this agreement in accordance with any and all regulatory and accreditation standards applicable to facility, including without limitations, those requirements imposed by the Joint Commission of Accreditation of Healthcare Organizations (JCAHO). Contractor shall provide consultation, if requested, in the development of protocols, utilization review, peer review and risk management as required by JCAHO and other regulatory bodies. h. All contractor provided services must comply with all VHA mandates, directives, policies, and programs, and in accordance with VA policies and procedure and the regulations of the medical staff by-laws of the Omaha VA facility. 2. TASKS: a. Performs all aspects of autotransfusion in the intraoperative and perioperative settings b. Perform Buffy coat services/procedure c. Provide protocols and guidelines for transfusions d. Perform quality control tests and quality control assessments e. Provide proper documentation of all activities on the medical record. Initiates and maintains written records specific to the procedure. f. Provides information for update into VA computer patient system to access and enter patient information, etc. g. Provide services on a 24-hour, as needed basis, seven days a week, including holidays. h. Provides verbal and written documentation of the volume of blood and/or blood component returned to the patient to the anesthesiologist, surgeon, or nurses associated with the patient's care. i. Monitors the autotransfusion equipment when in use. j. Ensures pager working correctly and turned on to receive pages. k. Collects samples of the salvaged blood product for quality assurance purposes (platelet count, ABC, as indicated). l. Maintains skills and knowledge required to perform job. m. Performs other related duties as assigned. 3. DESCRIPTION OF PROCEDURES: a. Intraoperative Autotransfusion (cell saver) services are a procedure in which the blood shed during an operation is subsequently returned to the patient's own circulation. Autotransfusion is used on big cases such as total hip replacements, aortic aneurysm repairs, prostatectomys, etc., where blood is usually cross-matched. It is especially used in cases where pre-op donation is impossible or inadequate. Post-op or chest tube salvaging would not be done. Autotransfusion should reduce the number of blood transfusions required. Autotransfusion should reduce the risk of infection, transfusion reactions, cross-matching errors, decreasing the patient's own immune system, etc. b. Buffy Coat: Procedure in which an autotransfusion cell separator is used for the production of platelet rich plasma sequestration (PRP) from whole blood collected from patient via a venous or arterial catheter. This platelet rich plasma can then be manually concentrated into Platelet Growth Factor (AGF). 4. CONTRACTOR EMPLOYEE QUALIFICATIONS/DUTIES All contractor provided clinical perfusionists or autotransfusionists shall be certified by the College of American Pathologists (CAP), the American Board of Cardiovascular Perfusion or American Board of Clinical Autotransfusion. Contractor must be proficient in operation of perfusion/autotransfusion equipment provided by the VA. The contractor will be responsible for submitting all proof of certification for each individual performing service under this contract. . Health sciences education and experience in a healthcare setting such as blood bank, emergency room, intensive care or surgery is required. Relevant certification could include MT (ASCP), RN, CCP, MD, MLT, EMT, LPN, CNA, MA, Phlebotomist. Previous experience operating cell-salvage equipment highly desirable. . Demonstrate technical skills; ability to operate medical equipment. . Basic math skills. . Attention to detail. . Excellent interpersonal skills. Ability to work independently. . Ability to react quickly and accurately and work effectively in the surgical setting. Ability to conduct self in a calm, capable and professional manner in a volatile environment. . Available to work flexible hours in order to provide 24-hour, 7-day/week service. When on-call, must be able to arrive at the hospital within two hours of notification. . Must be able to arrive to provide service in all weather conditions. . Ability to lift 50 pounds, to push or pull up to 700 pounds (on wheels), and to work on feet for prolonged period of time. 5. LOCATION OF SERVICES: All services will be performed at the Omaha VAMC. 6. GOVERNMENT FURNISHED PROPERTY AND DISPOSABLES: The Omaha VAMC shall furnish all supplies and disposable items (including bowl set, reservoir and suction line) required for performance of this contract.
