- REVISED PRE-SOLICITATION NOTICE: The Department of Veterans Affairs (VA) on behalf of and as an agent of the Division of Strategic National Stockpile (DSNS) for the Centers for Disease Control and Prevention (CDC) will issue Request for Proposal (RFP) 797-DSNS-0713 for AMOXICILLIN 400MG/5ML SUSPENSION, POWDER in bottles of 100ML EA. The estimated quantities are 1,155,176 bottles. Delivery shall consist of one delivery per month for eight consecutive months as reflected in the delivery schedule below: 1st Delivery no later than January 15, 2008; 157,464 BTL 2nd Delivery no later than February 15, 2008; 157,464 BTL 3rd Delivery no later than March 15, 2008; 157,464 BTL 4th Delivery no later than April 15, 2008; 157,464 BTL 5th Delivery no later than May 15, 2008; 131,330 BTL 6th Delivery no later than June 15, 2008; 131,330 BTL 7th Delivery no later than July15, 2008; 131,330 BTL 8th Delivery no later than August 15, 2008; 131,330 BTL Offeror's must state the exact name (name that appears on label) by which the drug will be supplied and have a unique NDC number. If the offeror is not a manufacturer, a signed letter of commitment between the offeror and the manufacturer must be submitted with the offer. Solicitation No 797-DSNS-0713 will be electronically issued in commercial item format in accordance with FAR Part 12 on or about September 20, 2007, with a closing date of October 4, 2007.
