Description
The government intends to award a sole source contract for the following commercial services: The 30th Contracting Squadron, Vandenberg AFB, CA intends to award a sole source firm-fixed priced service contract to JP Blanchard. This acquisition has two CLINs to provide the following IAW the attached Statements of Work: CLIN 0001: Contractor to furnish all labor, tools, parts, materials, facilities, transportation, and supervision necessary to provide histopathology services at the 30 MDG. CLIN 0002: Contractor to furnish all labor, tools, parts, materials, facilities, transportation, and supervision necessary to provide pathology services at the 30 MDG. The driving distance from the lab to the 30 MDG can be no more than 1 hour. This combined synopsis and solicitation is prepared in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. The proposed contract is for services for which the Government intends to solicit and negotiate with only one source under the Authority of FAR 13.106(b). This notice of intent is not a request for competitive proposals. However, the Government will consider all proposals received within 5 days after date of publication of this synopsis. A determination by the Government not to compete this proposed contract based upon responses to this notice is solely within the discretion of the Government. Information received will normally be considered solely for the purpose of determining whether to conduct a competitive procurement This acquisition is subject to availability of funds. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-16 and DFARS Change Notice 20070327. North American Industrial Classification Standard (NAICS) 621511, Small business size standard ? $12.5M applies to this procurement. The period of performance will include a base year from 1 October 2007 to 30 September 2008 and 2 one-year option periods, extending through 30 September 2010; exercised at the unilateral right of the Government. STATEMENT OF WORK HISTOPATHOLOGY SUPPORT Non-Personal Services Histopathologist shall: provide diagnosis on tissue specimens. Diagnosis includes macroscopic gross description of specimens and microscopic diagnosis of tissue sections. Provide frozen section diagnosis on biopsy material. Provide cytological diagnosis on GYN and non-GYN specimens. Provide phone consultation on cases submitted for histological or cytological diagnosis. Provide detailed written reports. Provide representative slides for quality control and peer review by military histopathology centers. Slides must be accompanied by written report detailing histological and/or cytological diagnosis. When requested, provide phone reports of histological/cytological diagnosis upon completion of studies. Due to the immediate requirement for examination of frozen tissue biopsy material, the location of the laboratory providing this service must be within a 1-hour driving radius of the 30th Medical Group. The contracted Pathologist must have a current certification from the American Board of Pathology. It is the contractor?s responsibility to report (to the appropriate MTF staff member) all information necessary to assure hospital records can be maintained correctly, and there comply with the JCAHO, OSHA, CAP, CDC, AABB, HSI, and FDA health records requirement. Contracted Pathologist shall upon request provide quality data and information related to the services provided that support both the Performance Improvement program and the Risk Management process. Conduct a monthly review of our Quality Control and CAP surveys. Act as final authority when it comes to the approval of the studies, giving the go ahead to implement the new instrumentation/method. PATHOLOGICAL CONSULTING SERVICES Non-Personal Services STATEMENT OF WORK Review quality control records and quality assurance documentation Contracted Pathologist shall upon request provide quality data and information related to the services provided that support both the Performance Improvement Program and the Risk Management process. It is the contractors responsibility to report (to the appropriate MTF staff member) all information necessary to assure hospital records can be maintained correctly, and there comply with the JCAHO, OSHA, CDC, CAP, AABB, HSI and FDA health records requirement. The contracted Pathologist must have a current certification from the American Board of Pathology. Provide consultation to professional staff concerning pathological issues/problems by telephone or during the monthly in-house visits. Provide written monthly report to Director, Medical Laboratory Services. Provide consultation/review services via: A. One (1) scheduled two-hour visit per month B. Unscheduled visits to consult/evaluate clinical specimens as agreed to by the pathologist and the Director of Medical Laboratory Services or his/her designated representative. Response must be on a STAT or ?as soon as possible? basis as agreed upon by the pathologist and the Director of Medical Laboratory Services or his designated representative. The visit must provide timely evaluation for provider diagnosis, interpretation and treatment and ensure evaluation prior to any specimen degradation or decrease in clinical value. C. Evaluation of clinical specimens delivered to the pathologist by government procured conveyance as agreed upon by the pathologist and the Director of Medical Laboratory Services or his/her designated representative. Evaluations must be ?STAT? or ?as soon as possible? basis as agreed to by the pathologist and the Director of Medical laboratory Services or his/her designated representative. Evaluations must provide timely evaluation for provider diagnosis, interpretation and treatment. Evaluation must be completed prior to any specimen degradation or decrease in clinical value. D. Unlimited telephone consultations. Provide services at all times. Due to immediate requirement for examination of frozen tissue biopsy material, the location of the laboratory providing this service must be within 1-hour driving radius of the 30th Medical Group. Contracted Pathologist shall upon request provide quality data and information related to the services provided that support both the Performance Improvement program and the Risk Management process. Conduct a monthly review of our Quality Control and CAP surveys.
