Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format prescribed in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotes are being requested and a written solicitation will not be issued. This solicitation is being requested as a Request for Quote (RFQ); solicitation number is H92244-07-T-0102, as a Firm Fixed Price (FFP) type contract with fixed unit prices. The anticipated award date is on or about 14 September 2007. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular (FAC) 2005-17, effective 14 June 2007. North American Industrial Classification Code (NAICS) 611699 applies to this procurement. This procurement is a 100% Small Business Set Aside. The DPAS rating for this procurement is DO-C9. FOB point is Destination. Section B Schedule of Supplies/Services CLIN 0001 Military Fitness Evaluation Course 1 lump sum Number of Students: 40-45 Session of Performance: 24 September 2007 ? 28 September 2007 Section C Performance Statement of Work Course of Instruction: Contractor shall provide evaluation of physical performance factors that will aid as primary components of a tertilary decision-making process in the selelction and training of military personnel. The program and education will follow American College of Sports Medicine (ACSM) curriculum. Training objectives include: Task 1: Ascertain cardiovascular exercise function: individual will educate class on cardiac response to exercise from sea level to altitude. Will educate class and show examples of how to manage on-the-road workout program through scientific evidence-based methodologies. Task 2: Determine Postural Control abilities: Will observe postural capabilities with ech membe in the class and educate on dysfunction. Task 3: Determine Objective Functional Strength abilities: Will observe the core, ?pillar movements? and provide sound examples of development of core stabilization using proven methods on professional athletes. Task 4: Determine Functional Movement dysfunctional patterns: utilizing the Functional Movement Screen, participants will be evaluated for dysfunctional movement patterns and graded on their movement capabilities. Instructor and/or staff: -must have CSCS certification with minimum of 2 years experience -must train and rehabilitate professional athletes year-round -have evidence-based methodologies to establish outcomes (from tests) that are objective rather than subjective as it relates to outcomes -show past performance experience, s it relates to testing -have had prior research experience in biomechanics and have worked with Department of Defense and Special Operations military personnel -have assisted or reviewed ?combine? functional testing with the military, within the past year -have familiarity with NFL combine testing -will not publish or disseminate any data with regards to this observation without prior written approval of NSWDG Senior Medical Officer -will have experience with testing administration of large groups greater than 50 students Location: Naval Special Warfare Development Group, 1636 Regulus Avenue, Virginia Beach, VA 23461 Number of Students: There are approximately 40-45 students. The following factors shall be used to evaluate offers: technical competence, past performance, and price. Technical competence and past performance, when combined, weighs more than price. Offerors shall submit past performance information to include contract number, government agency point of contact and contact information, description of supplies/services provided, unit of issue, unit price, and extended price. Past performance shall be current and shall not exceed three (3) fiscal years before posting date of the combined synopsis/solicitation on FBO. (b) Options. The Government will evaluate offers for award purposes by adding the total price for all options to the total price for the basic requirement. The Government may determine that an offer is unacceptable if the option prices are significantly unbalanced. Evaluation of options shall not obligate the Government to exercise the option(s). (c) A written notice of award or acceptance of an offer, mailed or otherwise furnished to the successful offeror within the time for acceptance specified in the offer, shall result in a binding contract without further action by either party. Before the offer's specified expiration time, the Government may accept an offer (or part of an offer), whether or not there are negotiations after its receipt, unless a written notice of withdrawal is received before award. Except as specified herein, no order, statement, or conduct of Government personnel who visit the contractor?s facilities or in any other manner communicates with Contractor personnel during the performance of this contract shall constitute a change under the Changes clause in Section I. In the event the Contractor effects any change at the direction of any person other the Contracting Officer, the change will be considered to have been made without authority and no adjustment will be made in the contract price to cover any increase in cost incurred as a result thereof.
