- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Reference number F1B3R27241A001 is hereby issued as a Request for Quote. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-18 5 July 07. This acquisition is under North American Industry Classification Standards (NAICS) code 511210, STATEMENT OF WORK (SOW) STATEMENT OF WORK FOR THE INSTALLATION OF THE AUTOMATED AF FORMs 332, 813, AND 103 SYSTEMS AT ALTUS AFB The purpose of this Statement of Work is to automate and integrate Air Force Forms 332, 813, and 103 with Altus? GeoBase and Microsoft Outlook programs. This automation offer the following benefits: web-based input and coordination of all 3 AF Forms, map attachment and tie-in to the installation GeoBase system, automatic routing of the AF Forms using MS Outlook email, electronic archiving of the data to allow quick retrieval and status reporting. To achieve this automation, the following tasks need to be performed. Task 1 ? Identify Workflow Processes The contractor will travel to Altus AFB to host a kick-off meeting to describe the applications and identify and document the workflows associated with the following activities: ? The Work Request Review process using AF Form 332 ? The Environmental Impact Analysis Process (EIAP) review process using AF Form 813 ? The Work Clearance review (aka Digging Permit) process using AF Form 103 In addition to gaining an understanding of their work flow processes, the contractor will also collect site specific primary and secondary point of contact information (names, organizations, phone numbers, e-mail addresses) to be incorporated into each application. Task 2 ? Configure and Install the Automated AF Form Systems The contractor will take the information collected during Task 1 and use it to modify and configure the existing three application programs to meet the specific needs of Altus? various workflow process. For the automation of the AF Form 332, the contractor will configure the sequence and timing of review activities among the customer service desk, coordinating agencies, environmental review, estimation, and Work Request Review Panel organizations. For the automation of the AF Form 813, the contractor will configure the organization review process sequence, enter alternate reviewers for each environmental area, and set up final signature authorization. For the automation of the AF Form 103, the contractor will also configure the sequence of organization reviews, incorporate the methodology for site inspection, and set up final signature authorization. The contractor will then travel back to Altus AFB to install and configure the three application programs. These tasks include installing and integrating an updated GeoBase viewer, configuring Altus? Oracle GeoBase database server to accommodate the new applications, installing the three web-based applications, and testing system functionality. Task 3 ? Training Software training will be provided for designated Altus staff following system installation. The contractor will provide a total of 3 training session on the application programs - one training session for each of the 3 automated AF Form applications. The contractor will provide approximately 4 hours training for each application. These training sessions will be conducted in a ?Train-the-Trainer? format and it will be the responsibility of the Altus trainers to train other Altus users in the operation, update, and maintenance of the installed systems. Task 4 ? Follow up and Modification The contractor will provide an additional 20 hours of telephone support to Altus to answer any questions or address any system issues following installation and training. This telephone support will be available to Altus for 60 days following final system installation and training. Task 5 ? Monthly Status Reports The contractor will submit monthly status reports on the 7th workday of each month to the contracting officer and the Base POC. Deliverables: ? An installed automated AF Form 332, 813, and 103 Application Programs linked to Altus? GeoBase ArcIMS Map Services, Oracle database management system, and MS Outlook E-mail system. ? Computer Based Training Materials Acceptance Criteria The Altus POC concurrence and acceptance of the installed application shall constitute completion of the task. Period of Performance A period of performance is expected of at least 120 days. The contractor will complete all requirements, configuration, installation, and training work within 90 days of notice to proceed. The contractor will then provide an additional 30 day period of performance for post installation telephone support. (End SOW). All work shall be performed IAW SOW.
