Description

This is a notice of intent of the Center for Health Care Contracting (CHCC) at Fort Sam Houston, TX to award an Indefinite Delivery contract for non-personal services. To provide a qualified dentist provider network consisting of private civilian de ntist to perform routine and emergent dental treatment within their private offices as directed by a treatment plan provided by the Soldier Readiness Processing (SRP) dental station in order to remove a Reserve Component (RC) Soldier from Dental Class 3 de ployment status into a minimum of a Dental Class 2 deployment status IAW Health Affairs Policy: 02-011. The contractor shall be required to initiate a dentist provider network which is required to treat referred Soldiers on 01 April 2008 at 7 primary mobi lization sites. The Contractor shall create additional standardized dentist provider networks at any of 21 secondary sites within 60 days of a formal request by the DENCOM COR. The number of service members referred to the local provider network at any of the SRP dental stations approved to use the RC-MRP is completely dependent upon the number or RC Soldiers mobilizing through that station, the status of their dental readiness w hen they present to that dental station, and the SRP dental stations capabili ty to treat the RC Soldiers within their facilities. Primary site locations to be serviced include: Camp Atterbury, IN; Camp Shelby, MS; Ft. Benning, GA; Ft. Bliss, TX; Ft. Dix, NJ; Ft. Jackson, SC; and Ft. McCoy, WI. Secondary site locations to be servic ed include: Camp Roberts, CA; Ft Bragg, NC; Ft. Buchanan, PR; Ct. Campbell, TN; Ft. Carson, CO; Ft. Drum, NY; Ft. Eustis, VA; Ft. Gordon, GA; Ft. Hood, TX; Ft. Irwin, CA; Ft. K nox, KY; Ft. Lee, VA; Ft. Leonard Wood, MO; Ft. Lewis, WA; Ft. Polk, LA; Ft. Ri ley, KS; Ft. Sam Houston, TX; Ft. Sill, OK; Ft. Stewart, GA; Gowen Field, ID; and Schofield Barracks, HI. However, if the large mobilization troop surge occurs, the RC-MRP network must be capable of expanding to any one of 21 additional mobilization platfo rms. The contract performance period will be for a 6-month base period plus four 12-month options and one 6-month option period. Performance base period will begin 01 April 2008 through 30 September 2008, four 12-month option periods from 01 October 2007 through 30 September 2012 and one 6-month option period from 01 October 2012 through 31 March 2013. The NAICS code for this procurement is 621210 and the Small Business Size Standard is $6.5 million. The solicitation will be issued on or about 17 Octobe r 2007.
