Description: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a quotation is being requested and a written solicitation will not be issued. Solicitation number F2F3297229A902 is hereby issued as a Request for Quotation. The incorporated provisions and clauses are those in effect through Federal Acquisition Circular (Fac) 2005-19, Effective 17 Sep 2007 and Fac 2005-20 , Effective 06 Sep 2007 & Class Deviation 2005-o0001. The North American Industry Classification System (NAICS) code for this project is 611710 with a size standard of $6.5 Million, the SIC code is 8299. The contractor shall provide provide Distance Learning Job Site Training POC (technician) services for Hurlburt Field?s Education Services Office as statement in the Statement of Work. FOB: Destination for delivery and installation to Hurlburt Field, FL 32544. The following provisions and clauses apply to this acquisition and will remain in full force in any resultant award: FAR 52.212-1, Instructions to Offerors?Commercial Items (September 2006). It is amended to read: Offeror shall submit signed and dated offer to 1 SOCONS/LGCY, Attn: A1C Fausto Fernandez, 350 Tully St Bldg 90339, Hurlburt Field FL 32544-5810 NLT 26 September 2007; 1600 hrs CST. Submit signed and dated quotations on company letterhead or quotation form and marked with solicitation number F2F3267201A001. Offer may be faxed to 850-884-5372. Proposals may be accepted by email. The government reserves the right to evaluate proposals and award a contract without discussions with offerors. Offers shall be evaluated in accordance with FAR 52.212-2, evaluation of Commercial Items (Jan 1999) which is incorporated into this Request for Quote, with an addendum to paragraph (a) as follows: The following factors shall be used to evaluate offers: (i)technical capability of the service offered to meet the Government?s requirement; (ii)price; (iii) past performance . STATEMENT OF WORK: DISTANCE LEARNING (DL) SATELLITE DOWNLINK SITE/ TESTING OF COMPUTER BASED TRAINING (CBT) PERFORMANCE WORK STATEMENT 06 September 2007 Contractor shall provide site monitor services at a Distance Learning (DL) satellite downlink site in accordance with established directives and requirements listed below. Contractor will provide services for testing all Computer Based Training (CBT) as required. The contractor shall ensure a quality educational environment is maintained at all times. The contractor must maintain a close and continuing contact with the Education Center staff to ensure a high degree of professional understanding, cooperation, and liaison supportive to the mission of the Flight. Services will be one year during the period of 1 October 2007 to 30 September 2008 for the Base period plus a potential of four options in support of the Hurlburt Field Education Office. These services will be performed at the Hurlburt Field Education Office, Building 90220, 221 Lukasik Ave., Hurlburt, FL 32544 or within the confines of Hurlburt Field. 1. Minimum Experience: Contractor must have a minimum of one-year experience providing education or training to adults. Experience working with audio-visual equipment and computers. Experience working with teleconferencing equipment is desired but not required. A high school diploma or General Education Development (GED) equivalency certificate is required but college level education is preferred. 2. Definition of Session: A session is defined as 1 hour of time. Set-up for the sessions may begin as early as 0600 and as many as 40 sessions may occur during one week. 3. The service provider shall be available from 0600 to 1630, Monday thru Friday, on a need basis, times subject to change. Payment for each session scheduled and satisfactorily delivered will be made at the per session price stated in the contract. The service provider may be informed informally by the Education Serivces Officer or their representative. Cancellation of scheduled sessions shall be made at least 12 hours in advance when only one session is scheduled. 4. Requirements: a. Equipment 1. Sets up classroom(s) equipment for use for satellite, video, and computer-based or resident instructor delivery methods a minimum of one hour prior to beginning each scheduled broadcast. 2. Establishes audio connection and ensures monitor is set accurately prior to start of scheduled broadcast. 3. Ensures all equipment is working correctly. Maintains a log of equipment malfunctions. Notifies the Education Services Officer for service /repair of equipment. 4. Monitors each complete broadcast and as it occurs, report audio or video problem to origination site. 5. Records and maintains video recordings of instructional programs for student review. 6. Ensures equipment is secured. b. Facilities 1. Opens and locks classroom(s). 2. Ensures classroom(s) are set up to accommodate scheduled student load. 3. Ensures classrooms are neat, orderly, and clean before and after sessions. 4. Informs the Education Services Officer of facility problems requiring repair. c. Scheduling, Enrollment, and Student Administration 1. Pre-Broadcast a. Conducts orientation to include explaining building/classroom procedures and the use of the equipment, discussion of schedule, and to encourage student participation. b. Takes attendance. c. Completes class roster and forwards to appropriate offices. d. Ensures that the connection is made with audio and video. e. Distribute class materials (Study guides and information hand out). f.. No later than the end of the first hour of class, hand out the AECT Form 325 for students to verify accuracy and make any entries necessary. g. Pick up that copy of the AETC Form 325 and fax it immediately to the instructors at the studio. 2. During Broadcast a. Monitors students. b. Assists course facilitators and students in resolving equipment and student materials problems. c. Monitor the IVT signals (audio and video). d. Be sure the class leader understands her/his professional role in the class. e. Coordinate and frequently check with the class leader for any assistance the class may require during the training day?during off or on airtime. f. Ensure students are present by an attendance roster. g. Monitor the classroom to keep students focused on the presentation. h. Encourage the students to participate using the microphone. i. Notify the uplink (broadcast) site of any technical difficulties. 3. Post Broadcast a. Administers student attrition survey, makes recommendation on retention when needed, and forwards to appropriate office. b. Administers student critique and forwards to appropriate office. c. Distributes end-of-course surveys. d. If required, assists test examiner to administer assessment instruments. e. If required, assists test examiner to score assessment instruments. f. Mails all materials to course dictator or origination site. g. Distributes course completion certificates or diplomas. h. Ensure the students have their study guides, exams, and answer sheets. i. Administer and monitor the exam. That is, administer one version. Give the other exam as retest, if needed. j. Score the students' answer sheets. k. Secure the test and answer key. l. Package the used answer sheets and feedback results, along with the signed 325 and mail to the instructional development specialist from the originating broadcast site. IAW Air Force Instruction 36-2201 Developing, Managing, and Conducting Training Chapter 5-5.30. Site Coordinators d. Support Materials Distribution and Collection: 1. Distributes course materials, as required, to students and facilitator. 2. Maintains a log of course materials. e. General Instructions: a. Attire. While in the performance of their duties, must wear appropriate professional business attire with a neat and clean image. b. Emergency Medical Service. In the event of an emergency, dial 911. Emergency medical care is not available on Hurlburt Field. An ambulance unit will respond to 911 calls, and if necessary, transport a Contractor employee to one of the local hospitals. The Contractor shall reimburse the respective agency for these services. c. Invoicing. Shall be submitted using Wide Area Work Flow (WAWF). See the contract clause, ?WIDE AREA WORK FLOW DODAAC AND EMAIL DISTRIBUTION TABLE? for instructions.. d. Government furnished equipment. Distance Learning Technician will be provided access to the base (if required), and the Government will provide all necessary equipment to conduct the work. e. Badges: The service provider shall obtain the required employee badges and vehicle passes for access to Hurlburt Field 24/7. The service provider shall have their contractor badge with them at all times. The service provider?s badge and pass shall be returned to the Hurlburt Field Pass & ID Security Office upon cessation of employment. f. Local Area Network Access (LAN). The government will provide the contractor a LAN and E-mail account. This service shall be strictly for conducting official business. Individuals shall pass the Information Assurance (IA) course and in accordance with AFI 33-119, Electronic Mail Management and Use, paragraph 9.2.5, contractor personnel using unclassified automated information systems that have access to sensitive information shall possess, at a minimum, a National Agency Check or Entrance National Agency Check in accordance with DoD 5200.2-R, Personnel Security Program or screening in accordance with AFI 31-501, paragraph 3.27, Personnel Security Program Management. Additional DL duties ? Testing for all CBT, as required ? The use of the Answer Sheet Manager Program for scoring test disks and any problems associated with the program, or other similar programs as required ? Responsible for the Electronic Answer Sheet Transfer Procedures (FTP), to include reporting of equipment failure to the Air Force Institute for Advance Distributed Learning (AFIADL) (or other appropriate sites) ? Responsible for all associated administrative work, tracking students, safeguarding disks, etc. ? Responsible for all course glitches and required fixes. Requires computer/internet knowledge ? Responsible for setting up the additional DL computers as a computer lab with the computers set up as lesson computers for those who don't have access in the duty section or at home. ?
