Description
This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation #W9124J-07-T-0083 is issued as a Request for Quotation. The North American Industrial Classification System Code (NAICS) 518210 size standard of $23.0 million applies. The proposed solicitation is for 100% small business set aside. The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular FAC 2005-15 dated 13 February 2007. The Southern Region Contracting Center-West (SRCC-W) intends to award on a firm fixed price contract to provide all the necessary services to include personnel, services, parts, material, tools, equipment and vehicles required to provide high secure fax se rvices to send and receive fax document between /Community Based Health Care Organizations (CBHCOs) their respective Managed Care Support Contractors (MCSCs) and patients for The Great Plains Regional Medical Command for two companies located at Arkansas C BHCO @ Camp Robinson, North Little Rock, AR and Utah CBHCO@ Camp Williams, Riverton, UT. The contractor shall provide a system that allows transmission of information from the CBHCOs using existing desktop computers and web-based internet capabilities. I nformation being transmitted and received will contain Personal Health Information (PHI); therefore, service must be in compliance with requirement set forth by the Health Insurance Portability Accountability Act (HIPAA). The contractors system must be a ble to function across the Internet and military network, be simple to use and maintain, and require no Government System Administration support staff. The contractors system must be deployable to approximately 40 MTFs comprised of approximately 200 end users. System must be flexible enough to support increase or decrease requirements for either or both CBHCOs and end users given the requirements of the Medical Command. The proposed system must be functional a minimum of 12 hours per day, 365 days per y ear during the period 6:30 AM to 6:30 PM in each local time zone. Contractors technical support staff must be available to trouble-shoot system problems Monday-Friday during the period 7:30AM to 4:30PM in each local time zone. No on-site contractor staff is expected. The contractor shall process an estimated 300,000 pages of facsimiles per year. The services to be provided are commercial within the meaning of FAR Clause 52.212-1. Period of performance shall be for five years with annual reviews during the anniversary month. Statement of Work CBHCO AR/UT HIPAA Compliant Electronic Fax Solution 1.1 GENERAL: The Great Plains Regional Medical Command requires a high secure, facsimile (fax) serv ice to send and receive fax documents between Community Based Health Care Organizations (CBHCOs), their respective Managed Care Support Contractors (MCSCs) and patients. The contractor shall provide a system that allows transmission of information from th e CBHCOs using existing desktop computers and web-based internet capabilities. Scope of Work: The principle objective of this performance work statement (PWS) is to provide GPRMC CBHCOs with electronic fax capability to send and receive fax documents in a secure, HIPAA compliant electronic environment. 1.2.1. The electronic fax service will ensure that the CBHCOs can send and receive electronically sec ure faxed documents. The majority of fax volume will occur between the CBHCO and the TriCare Managed Care Support Contractor and their associated network health care providers offices. This does not however, limit the requirement to provide electronic f ax services between the CBHCO and other locations. 1.2.2.The Government requires flexibility in the quantity, establishment, termination and relocation of electronic fax accounts. The number of fax accounts at each participating CBHCO is contingent upon the number of assigned personnel, expected referrals and subsequent fax volume. Since CBHCOs are subject to variation in workload and personnel turnover due to permanent change of station, deployments, etc., the origination or termination of the initial account will be a one-time fee and not subject to recurrence when associated with variation in workload and/or personnel turnover. 1.3.Hours of Operation and On-call Support: No on-site contractor staff is expected. The electronic fax product and associated off-site equipment must be functional a minimum of 12 hours per day, 365 days per year during the period 6:30 AM to 6:30 PM, in each local time zone. Con tractor technical support staff must be available to trouble-shoot system problems Monday-Friday during the period 7:30AM to 4:30PM. 2. Contractor Furnished Equipment Materials and Services. The Contractor will provide all the necessary equipment to per form the electronic fax functions with the exception of desktop computers, scanner and other miscellaneous equipment available within the Government facility required to access the contractors (business associates) electronic fax service. 3. Technical Requirement. The electron fax solution must: 3.1 Be web-enabled to meet Department of Defense (DoD0 and security requirements. 3.2 Be Federal Information Processing Standard (FIPS) 140-2 level 3 cryptographic certification for an open Secure Shell Layer (SSL) architecture 3.3 Provide a secure, encrypted end-to-end connection supported by authentication and server SSL certification. 3.4 Support https or http and Virtual Private network (VPN) configurations. 3.5 Support Light Weight Directory Acce ss Protocol (LDAP) in accordance with (IAW) military healthcare systems (MHSS) requirements. 3.6 Be 508 compliant (information can be obtained at http://www.section 508.gov/index.cfm) 3.7 Ensure data encryption during data transmission and storage of PHI data outside of source system IAW MHS management of PHI data and HIPAA requirements. 3.8 Have an extensive audit capability IAW Evaluation Assurance Level -3 (EAL3) certification. 3.9 Support interactive, batch or scheduled communication. 3.10 Be Window NT/2000/XP compatible. 3.11 Provide 50 MB of secure Account/mailbox capacity for each end user and 150 MB secure Account/mailbox for each MTF. 4. Functional Requirements: Additional requirements for an acceptable fax solution are: 4.1 Any authorized user shall be capable of sending and retrieving an electronic fax from their Internet connected computer workstation. 4.2 The electronic fax solu tion shall be capable of seamlessly sending a variety of digital file types to include the following; doc, xls, pdf, tif, jpg, txt. Several different electronic documents (files) shall be able to be sent as a single fax. The capability shall also exist t o seamlessly fax a scanned paper document. 4.3 The format of incoming digital fax documents must include the capability of storing a Portable Document Format (PDF) or equivalent of the incoming fax document. 4.4 Authorized users shall be granted accounts and privileges in accordance with DOD and regulations. The actions of authorized users of the electronic fax solution shall be logged in such a way as to be individually identifiable and attributable. 4.5 As used of this electronic fax solution may include unclassified, sensitive PHI, every step of the process for both incoming and outgoing faxing shall be documented to be incompliance with HIPAA. 4.6 Whenever digital images of faxed or to be faxed doc uments are stored on Internet accessible devices, those digital images shall be protected from unauthorized use or viewing in compliance with HIPAA. 4.7 If digital images of faxed or to be faxed documents are stored on Internet accessible devices, those digital images shall be completely eradicated from those devices either when completely processed for the sender or receiver, or after a predetermined period of time that is acceptable to the Government. By completely eradicated, it is meant that by no reasonable means could those images be restored from those Internet accessible devices. 5. Privacy of Protected Health Information: 5.1 Definitions. As used in this section. 5.1.1 Individual has the same meaning as the term individual in 45 Code of Federal regulation (CFR) 164.501 and shall include a person w ho qualifies as personal representative in accordance with 45 CR 164-502(g). 5.1.2 Privacy Rule means the Standards for Privacy of Individually Identifiable Health Information at 45 CFR part 160 and part 164, subparts A and E. 5.1.3 Protected Health Information has the same meaning as the term protected health information in 45 CFR 164.501, limited to the information created or received by the contractor from or on behalf of the Government. 5.1.4 Required by Law has the same meaning as the term required by law in 45 CFR 164.501. 5.1.5 Secretary means the Secretary of the Department of Health and Human Services or his/her designee. 5.1.6 Terms used but not otherwise defined, in this Contract shall have the same meaning as those terms in 45 CFR 160.103 and 164.501. 5.2 The contractor agrees to not use or further disclose Protected Health Information other than as permitted or required by the contractor as required by law. 5.3 The contractor agrees to use appropriate safeguards to prevent use or disclosure of the Protected Health Information other than as provided for by this contract. 5.3.1 The contractor agrees to implement administrative, physical, and technical safegua rds that reasonably and appropriately protect the confidentiality, integrity, and availability of the electronic protected health information that it creates, receives, maintains, or transmits on behalf of the covered entity. 5.4 The contractor agrees to mitigate, to the extent practicable, any harmful effect that is known to the contractor of a use or disclosure of Protected Health Information by the contractor in violation of the requirements of this contract. 5.5 The contractor agrees to report to the Government any use or disclosure of the Protected Health Information not provided for by this contract. 5.6 The contractor agrees to ensure that any agent, including a subcontractor, to whom it provides Protected Health Information received from, or created or received by the contractor on behalf of the Government, agrees to the same restrictions and conditi ons that apply through this contract to the contractor with respect to such information. 5.7 The contractor agrees to provide access, at the request of the Government, and in the time and manner designated by the Government to Protected Health Information in a Designated Record Set to the Government or, as directed by the Government to an Indi vidual in order to meet the requirements under 45 CFR164.524.
