Description
Request for consideration of a contract for: Sharps Management Program. SCHEDULE: The performance period of this contract is October 1, 2007 through September 30, 2008 with Four (4) One (1) Year Options. SERVICES ESTIMATED QTY UNIT TOTAL PRICE TOTAL 1. Furnish : Sharps Management 1 yr. $ _________ Services October 1, 2008 through September 30, 2009 2. Furnish : Sharps Management 1 yr. $ _________ Services October 1, 2011 through September 30, 2012 Justification for request: To enhance patient and staff safety by reducing the possibility of needle sticks. To increase our use of reusable containers thereby reducing the quantity of Non-biodegradable materials we introduce into the environment (Bio systems containers can be used up to 500 times). Possibly reduce the cost of our current program. Statement of Work Required: " Facility to be serviced by a badged and uniformed technician. " Pick ups to be done using a fully permitted vehicle on a predetermined scheduled day. " Clean, decontaminated, reusable sharps containers will be unloaded from the vehicle and stored in a designated storage area. " Contractor will be capable of proving separate, secure storage if needed. " The facility will be surveyed, a service schedule will be determined and followed when servicing the sharps containers throughout the facility. " Servicing containers means to pick up used sharps containers and replace them with clean, decontaminated containers. " The technician will proceed with the appropriate transport equipment, or cart, and service containers as detailed in the container location survey for each unit or area. " Upon arrival in each area or unit to be serviced the technician will report to the nurse's station, or other designated area, to announce their arrival and receive any special instructions. " The technician will comply with the facility's infection control procedures for entering and exiting isolation rooms or restricted areas. " Service will be provided in sequential order to insure no areas are missed. " Technician shall knock before entering any room and identify him/herself as the technician, and the purpose for entering that room. (They should always exit the room by thanking the person in that room) " The fill level of the container will be checked by looking through the view window. Containers should be removed when they are one half full. " Before removing any container from it's mount or location, it shall be closed and sealed in a manner that will prevent spillage, or unauthorized access to it's contents. All cabinets or brackets must be locked before exiting the room. " Carts or containers will never be left unattended unless in a secured area (i.e. the lab). " The technician will notify the COTR if there are any areas that you are unable to service due to occupancy or other reasons. " The technician will also notify the COTR if any repairs are required. " Transport equipment or carts will be returned to the storage location while waiting pick up and transportation to the disposal facility. " Prior to transportation, the shipment of containers will be aggregated and each used container will be counted and labeled in accordance with all Federal and State regulations. " A manifest or tracking document will be prepared identifying the facility that generated the waste, the facility transporting the waste, the date, and the number of containers that are to be transported as well as other information required by State and Federal regulations. " A signed copy of the tracking document will remain with the customer and the other copies are to be taken with the driver or technician. " After the tracking document has been signed the sharps containers will be loaded onto the vehicle for transportation to the treatment facility. " Any containers with visible damage will be permanently marked so upon return to the processing facility the container will be removed from circulation and destroyed. " Bldg 44 will be assigned a separate site number and billed separately from the main hospital facility. " Each quarter a detailed report will be sent to the COTR via E-mail, listing the site serviced, service date, manifest number, qty, weight, and description of the container serviced on that date.
