Description
CATERED MEAL SERVICES STATEMENT OF WORK (SOW) FY08 UTA WEEKENDS 194TH MSG SERVICES 101 41ST DIVISION WAY CAMP MURRAY, WA 98430 STATEMENT OF WORK DESCRIPTION OF SERVICES. The contractor shall furnish all services, materials and transportation necessary to provide prepared meals at the 143rd CBCS, Air National Guard (ANG) 6736 Ellis Avenue South, Seattle, WA 98108-3597 from November 3, 2007 through September 30, 2008 for the scheduled Unit Training Assemblies (UTA) weekends in accordance with the following specifications and schedules. 3-4 NOV 07 1-2 DEC 07 5 - 6 JAN 08 2 - 3 FEB 08 1 - 2 MAR 08 5 - 6 APR 08 3 - 4 MAY 08 7 - 8 JUN 08 12 - 13 JUL 08 2 - 3 AUG 08 6 - 7 SEP 08 A. Contractor will furnish all cooked meals and deliver prepared meals in accordance with the SOW. They will provide a meal with a minimum choice of two meat entrees, one vegetable, one starch, gravy, salad bar, bread, fresh fruit, milk and desserts. Diff erent meals will be provided each day. (See attached sample menus.) " One meat entree will be chicken, pork or fish on each day. " No less than one vegetables and one starch will be provided for each guest. " Contractor will provide servings of fresh milk for approx. 50 - 55% of estimated patrons each day. Fresh 1% or 2% white milk will be the primary milk; however, chocolate milk will be authorized. Milk will be provided in half-pint cartons. B. Contractor will provide all condiments in individual serving packets. All food items will be served from disposable shallow pans (4 inch) to include disposable serving utensils. Hot foods at will be kept at 140-160 degrees Fahrenheit and cold foods at 33-40 degrees Fahrenheit. Once delivered all meals will remain the property of the Govt. The contractor shall take necessary measures to ensure the projected meal count for servings is met as noted in paragraph C. Contractor will be notified of meal co unt changes 72 hours prior to the UTA. C. For each meal period there will be a guaranteed minimum of 75 meals not to exceed 120 meals. Payment will be made for each meal served. Pricing for subject contract will be per meal. Invoices will be submitted once the UTA has been completed each mon th to the appropriate finance office. D. During UTA weekends, the contractor will be responsible for providing and setting up the serving line no earlier than 10:00 a.m. and no later than 10:45 a.m. on both days. The lunch meal will be ready for serving NLT 11:00 a.m. unless otherwise direct ed. E. Disposable, cups, plates, and eating utensils will be provided by the Government. The contractor will provide all required meals to include serving pans and serving utensils. F. Contractor will provide a copy of the UTA menu to the 194th SVS no later than the Monday prior to the scheduled UTA. G. Food shall be purchased from approved vendors in accordance with the FDA Food Code guidelines. A list of food sources must be provided upon request. Contractor will comply with all FDA Food Code guidelines. Contractor will provide copies of Washington State Licensing Certificates prior to start of contract. Public Health personnel must be allowed to conduct sanitary inspections in accordance with current FDA code guidelines as required.
