- Anticipated Issue Date: October 4, 2004. Anticipated Closing Date: 19 October 2004. The Defense Commissary Agency has a requirement for Linen/Mat Rental/Cleaning/Exchange Services for 31 commissaries located in the Western Region. The states covered by this requirement include: California, Nevada, Arizona, Utah, and Washington. The items to be serviced under this Request for Proposal (RFP) include butcher coats, shirts, pants, bib aprons, meat handling gloves, colored smocks, cobbler aprons, bar towels, various sizes of mats, and dust & wet mops. Quantities of each item will be identified in the RFP, by commissary. The contractor shall be required to provide the items in a variety of sizes. The contractor shall be required to pick up from the commissary a minimum of once each week all soiled garments, mats, and mops, and drop off clean replacement garments, mats, and mops at the same time. The performance for the 31 commissaries will be 1 December 2004 through varied expiration dates, depending on the number of awards made. The base period starts 1 December 2004 and can end between 8 and 12 months. The first option period will be for 12 months beginning with the next day following the ending date of the base period. Each Option period thereafter will be for 12 months following suit, for a total of four one-year options. The NAICS code is 812332 and the business size standard is $12 million. This acquisition is being issued as unrestricted with the exception of 3 stores that are Small Business Set Aside (SBSA). This RFP will be available on www.fedbizopps.gov under 'Other Defense Agencies' (ODA). Potential Offerors are encouraged to use this web site to obtain a copy of the RFP in lieu of requesting a paper copy. Request for paper copies must be in writing and mailed to Defense Commissary Agency, Attn: CBU CICDS (B. VanDoren), 1300 E. Avenue, Fort Lee, VA 23801-1800, e-mailed to barbara.vandoren@deca.mil, or faxed to (804) 734-8669. All requests must include the RFP number, requestor's name, address, phone, fax number and e-mail address. By submitted an offer, the offeror acknowledges the Government requirement to be registered in the Central Contractor Registration (CCR) Database prior to award of any contract or purchase order.
