- The Sierra Pacific Network Consolidated Contracting Activity (CCA) is issuing a Commercial Item Request for Proposals (RFP) in accordance with FAR 12.203 to purchase HIV, HCV and Sample Extraction reagent tests, testing equipment and service of equipment on a cost-per-test basis with no consumables. This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; a proposal is being requested and a written solicitation will not be issued. This procurement is a sole-source justified action as Abbott Laboratories, Inc. is the only firm available to provide the required items, equipment, and service on a cost-per-test basis. The simplified acquisition procedures of FAR Part 13 will be used in conjunction with commercial items procedures of FAR Part 12. The NAICS code is 424210, with a small business size standard of 500 employees, and the Product Service Code is 6515. The solicitation number for this RFP is VA-261-07-RP-0292. The following provisions and clauses apply to this solicitation: FAR 52.212-1 Instructions to Offerors - Commercial Items (SEP 2006) and addendum; FAR 52.212-3Offeror Representations and Certifications - Commercial Items (SEP 2007), Offerors must include a completed copy of the provision with its offer; FAR 52.212-4 Contract Terms and Conditions - Commercial Items (FEB 2007); FAR 52.212-5 Contract Terms and Conditions Required to Implement Statutes or Executive Orders - Commercial Items (SEP 2007), For the purposes of this clause items 1, 3, 7, 16-24, 28, 29, and 35 are considered checked and apply; FAR 52.232.18 Availability of Funds (APR 1984); FAR 52.247-34 F.O.B. Destination (NOV 1991); VAAR 852.270-1 Representatives of Contracting Officers (APR 1984); and VAAR 852.270-4 Commercial Advertising (NOV 1984). Solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-20. This is a best-value procurement. Small businesses, especially veteran-owned and service-disabled veteran-owned small businesses, are encouraged to contact their nearest Procurement Technical Assistance Center (PTAC) if assistance is needed in preparing their proposal.
