Description
This is a combined synopsis/solicitation for commercial services prepared in accordance with the format in FAR Subpart 12.6, as supplemented with additional information included in this notice for the South Texas Veterans Health Care System. This announcement constitutes the only solicitation. Quotes are being requested and a written solicitation will not be issued. This procurement is being issued as a Request for Quotations (RFQ), and the reference number is for an annual service contract for the period of Date of award thru September 30, 2008 for various cardiovascular equipment systems. Contractor will provide all parts, labor, transportation, supervision and insurance to perform a full coverage maintenance service consisting of 2 periodic preventative maintenance calls as well as all emergency calls per year for the following items: Sarns Heart Lung Machine Model #8000: consisting of: (1 EA) Sarns heart lung machine model# - 8000. Which consists of the following subassemblies on the cart: 16401 modular base 5 pump / 1580 /, 16402 modular pump 115V / 13334, 16402 Modular Pump 115V / 13338, 16402 Modular Pump 115V / 13339, 16402 Modular Pump 115V / 13340, 16422 Battery / 1514, 16401 Modular Base 5 Pump / 1580, 16402 Modular Pump 115V /, 6382 Flow Sensor /, 164268 Delphin Manual Drive /, 9490 Delphin Battery /, 16404 Safety Monitor /, 5773 UAS Air Sensor. (1 EA) Sarns perfusion system model# - 8000 consisting of: 16401 Modular Base 5 Pump, 16402 Modular Pump 115V, 16402 Modular Pump 115V, 16402 Modular Pump 115V, 16402 Modular Pump 115V, 16402 Modular Pump 115V, 6382 Flow Sensor, 164268 Delphin Manual Drive, 9490 Delphin Battery, 16404 Safety Monitor, 5773 UAS Air Sensor, 4415 TCM2 TUV 115V. (1 EA) Sarns battery module, 8k Model # -16422. (2 EA) Sarns Cardioplegia monitors model# - 8k Cardioplegia. (2 EA) Sarns Arterial monitor model# - 8k arterial. (2 EA) Sarns Terumo centrifugal system models#- 6379. (2 EA) CDI 500 units consisting of (2 EA) 500 AHCT Monitors, and (2 EA) 540 Calibrators. For the following equipment, Contractor will provide all parts, labor, transportation, supervision and insurance to perform a full coverage maintenance service consisting of 1 periodic preventative maintenance call as well as all emergency calls per year for the following items: (3 EA) Fresenius Continuous Auto Transfusion system model# - C.A.T.S. Routine maintenance hours will be from 8:00 to 4:30pm Monday thru Friday. All maintenance inspections will be scheduled and coordinated with facility. Emergency response calls will be no more than four (4) hours with (2) hour minimum telephone response call back. All service technicians must have certification for service and/or repair from manufacturer on above identified equipment. Alterations made to system will be made only with the consent of the Government. ALL OFFERORS ARE TO INCLUDE A COMPLETED COPY OF THE PROVISIONS AT 52.212-3, OFFEROR REPRESENTATIONS AND CERTIFICATIONS COMMERCIAL ITEMS WITH OFFER.
