- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W912EQ-08-Q-0003 and is being issued as a Request for Quote (RFQ). This procurement is 100% Small Business Set-Aside. The associated NAICS code is 532412 with 500 employees. All responsible sources may submit a quotation, which shall be considered by the agency. Quotes may be submitted in hard copy or electronically, either through regular mail, email, or by facsimile. Regardless of method of delivery, it is the of ferors responsibility to ensure receipt of quotes by the time and date stated herein. Contractors shall submit a lump sum quotation to include all charges. SECTION A DESCRIPTION/SPECIFICATIONS/WORK STATEMENT A-1. Scope - The work to be done under these specifications consists of furnishing all plant, labor, equipment and materials, and performing all operations in connection with the delivery of two (2) 25 yard off road dump trucks. The dump trucks will be leased for a period of 30 days with 2 options fo r an additional 30 days. SECTION B DELIVERIES Deliveries Quick turnaround is necessary in order for the Government to fulfill its mission. Therefore, the items to be supplied under this purchase order shall be delivered to the job site beginning on or after 29 October 2007. The trucks shall be delivered to a location in Krotz Springs, Louisiana. Directions will be provided at time of award. Prior to shipment, the Contractor shall coordinate delivery with the following personnel. Primary Matt Pierce, 901-544-0950 or 901-573-4180 SECTION C MEASUREMENT and PAYMENT Payment. Payment will be made at the purchase order unit price for the quantity of acceptable material delivered. No payment shall be made for supplies replaced due to the Contractors fault or negligence. SECTION D RENDITION OF INVOICES AND VOUCHERS D-1. Submission of Invoices. Invoices shall be submitted in triplicate, typewritten or with pen and ink. Terms of payment must be stated, including cash discount, if any. D-2. Routing of Invoic es and Vouchers. Invoices or vouchers shall be sent to the following address.
