- Description
- The Contracting Department, Fleet and Industrial Supply Center (FISC), Norfolk, VA, 23511-3392 intends to negotiate a sole source Firm Fixed Price contract and will utilize procedures under FAR Subpart 13.5, Test Program for Certain Commercial Items with AMO USA, Inc., 1700 East St. Andrew Place, Santa Ana, CA 92705 for Naval Medical Center, 54 Lewis Minor Street, Portsmouth, VA 23708-2297. The Contractor shall provide two hundred (200ea) Custom Vue Vision Key Treatment Card (10 treatments per card) PN: 0030-4835 to include: (10ea) CustomVue License Fee, (10Tx) CustomVue with High Myopia Treatment Card, (2Tx) CustomVue Enhancement Card, (2Tx) PreVue Card and (2ea) PreVue Lens, Fifty (50) Star S4 Lasik/PRK Cards (10 Treatments per card), PN 0030-3551-11, and Five hundred (500ea) License Fee for Star S4 Lasik/PRK Cards. These cards will be used with Government owned STAR S4 IR Excimer Laser Correction System manufactured by AMO USA, INC. A complete RFQ package will be available on or about 27 October 2007 by downloading the documents at the Neco Website as follows: http:// www.neco.navy.mil. The North American Industry Classification System (NAICS) code for this acquisition is 334510. Potential contractors are hereby notified that the solicitation and subsequent amendments to the solicitation will ONLY be available for download at the above site; this office will no longer issue hard copy solicitations. By submitting an offer, the vendor is self-certifying that neither the requestor nor the principal corporate officials and owners are currently suspended, debarred, or otherwise ineligible to receive contracts from any federal agency. Failure to do so may represent grounds for refusing to accept the offer. By submission of an offer, the contractor acknowledges the requirement that a prospective awardee must be registered in the CCR database prior to award, during performance, and through final payment of any contract resulting from this solicitation, except for awards to foreign vendors for work to be performed outside the United States. Lack of registration in the CCR database will make a contractor ineligible for award.
