- VA-257-08-RP-0018 - Amendment #1 - Adding Option Years SCHEDULE OF SUPPLIES/SERVICES AND PRICES/COSTS Contractor shall provide all material and basic requirements for reading cardiology Electrophysiology Pacing (EP) event monitors at the VA North Texas Healthcare System, 4500 S. Lancaster Road, Dallas, Texas. These requirements include providing looping and non-looping event monitors. In addition the contractor must be able to receive transmissions for patients 24 hours a day 7 days per week and be able to notify the VA staff of abnormal transmissions at all times. Contractor must have a local representative. Services will include the patients being able to return the monitor directly to the contractor by mail. Electronic (e-mail) reports must be available. This contract is for a Base year with (2) two option years for a total of 3 years. OPTION YEAR ONE: 10/01/08 THROUGH 09/30/09 ITEM DESCRIPTION OF QTY UNIT UNIT AMOUNT NO. SUPPLIES/SERVICES PRICE 1 **BILL ONLY** Standard estimated amount of 30 day event monitoring service of outpatients in Cardiology/Electrophysiology Pacing Service. The monitors are assigned per consultation requests received for each patient requiring this service. 300 EA __________ __________ OPTION YEAR TWO: 10/01/09 THROUGH 09/30/2010 ITEM DESCRIPTION OF QTY UNIT UNIT AMOUNT NO.
