Description
The MEDCOM Health Care Acquisition Activity, Center for Health Care Contracting, Fort Sam Houston, Texas, intends to award a Single Award, Personal Services contract to provide 13 Community-Based Health Care Organization (CBHCO) Licensed Clinical Soc ial Workers (LCSW), to perform services as part of the Army wide Behavioral Health (BH) support of Soldiers and Families at CBHCO Army medical clinics located in Boston, MA; Redstone Arsenal, AL; Little Rock, AR; Salt Lake City, UT; Virginia Beach, VA; Roc k Island, IL; Orlando, FL, Sacramento, CA and Fort Buchanan, Puerto Rico. LCSWs shall provide clinical assessments, treatment, case management referral, and education according to their professional discipline, Army Medical Department standards, and the D OD directed Post-Deployment Health Evaluation and Management (PDH-CPG) program. The contractor shall provide these services as part of the multidisciplinary health care team to patients who are eligible beneficiaries of the Medical Health Care System. Co ntract service providers will also provide BH screening, assessment, and direct care, specialty referral and longitudinal BH care management to deployed Service Members and their Families with conditions and concerns related to their deployment. Prospecti ve offerors in order to receive an award must be registered with the Central Contractor Registration Program. The base period of performance for any contract awarded as a result of this solicitation is expected to start 1 February (with a Transition Perio d for credentialing and privileging) through 31 March, with full performance beginning 1 April 2008 through 30 September 2008, all dates inclusive, and four 12-month option periods from 1 October 2008 through 30 September 2012. The solicitation is expecte d to be released on or about 25 October 2007 and will be available for downloading at https://www.fedbizopps.gov/.
