- INSTRUCTIONS TO OFFERORS 1. The solicitation comprises the requirements of RFP VA-246-08-RP-0008 for Hazardous Chemical Waste Disposal in accordance with all terms and conditions as set forth, for the Department of Veterans Affairs Medical Center, 508 Fulton Street, Durham, NC, 27705 (hereinafter referred to as the VA or Government), as specified herein. 2. Small Business concerns are invited to submit technical, past performance, and separate pricing information in accordance with the Request for Proposal (RFP). The Department of Veterans Affairs (VA) anticipates that the RFP will result in firm-fixed priced service contract for a 1-year base period and four (4) 1-year option periods. Options will be exercised at the discretion of the Government. 3. Small Business concerns are encouraged to review the requirements for proposal submission thoroughly. The technical, past performance, and cost information must be submitted separately in accordance with the instructions contained in Chapter E of the RFP so that evaluation may be accomplished concurrently and independently. If your firm is submitting restrictive information, please include the following statement on page 1 of your proposal: "This data, furnished in response to RFP VA-246-08-RP-0008 will not be disclosed outside the Government or duplicated, used, or disclosed in whole or in part for any purpose other than to evaluate the proposal, provided that if a contract is awarded to the offeror as a result of/or in connection with the submission of the data, the Government will have the right to duplicate, use, or disclose this data to the extent provided in the contract. This restriction does not limit the Government's right to use information contained in such data if it is obtained from another source without restrictions. The data subject to restriction is contained on pages # through # ." 4. All questions concerning the RFP must be directed to the Contracting Officer via e-mail at Marsha K. Harrington-Evans.va.gov. Questions not directed to the Contracting Officer will not be considered and will not be answered. You are cautioned that no contacts are to be made with any other VA employee concerning this acquisition. 5. The date for receipt of initial proposals is December 4, 2007 , 8 a.m. (est). Proposals must be mailed to the address below: Department of Veterans Affairs VA Mid-Atlantic Health Care Network ATTN: MARSHA K. HARRINGTON-EVANS CONTRACTING OFFICER - VISN 6 300 W. Morgan Street #700 Durham, NC 27701 NO faxes or electronic submissions will be accepted. 6. Small Business concerns are instructed to provide the following: 1. Complete/Sign and return one (1) copy of the offer, page 1, SF 1449. 2. Complete/Sign and return one (1) copy of Contract Administration Data, page 4. 3. Complete/Sign and return one (1) copy of Supplies/Services and Prices/Costs, pages 5-24. 4. Provide one (1) copy of your Professional Liability insurance certificate, page 34. 5. Provide a Technical Proposal in response to the Evaluation - Commercial Items factors listed, page 46. 6. Complete/Sign and return one (1) copy of Offeror Representations and Certifications, pages 46-57. 7. Deliveries to other than the above will be considered as late offers unless redelivered to the above address prior to the hour and date set above for the receipt of initial proposals. 8. The Government reserves the right to reject any proposal at any time after receipt and to conduct negotiations to the extent the Government deems necessary and appropriate. However, your proposal should be submitted on the most favorable basis as to price, delivery or time for completion and other factors since the Government may elect to make an award without further discussions or negotiations. Proposals submitted in response to the RFP will not be returned, but will be retained by the VA as official records.
