Description
This is a combined synopsis/solicitation for commercial services prepared IAW the format in FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are bein g requested and a written solicitation WILL NOT be issued. The solicitation W901UZ-08-T-0002 is issued as a Request for Quotations. The solicitation document, incorporated provisions and clauses are those in effect through Federal Acquisition Circular 200 5-21, dated 11/7/07. This procurement is set aside for small businesses and issued under NAICS code 611699, small business size standard of $6.5 million. The North Dakota Army National Guard is requesting quotations for the procurement of Army Combat Lif esaver Course Instruction. The successful vendor will be required to teach (3) 3 day Combat Lifesaver Courses at (2) locations in North Dakota. The first course will be conducted in Bismarck, North Dakota 58501 from 7-9 January 2008. The second course w ill be conducted 11-13 January 2008 in Fargo, North Dakota 58102. The third course will be conducted 14-16 January 2008 in Fargo, North Dakota 58102. Quotes are due in by 12PM central standard time on December 6, 2007. Offers will be evaluated as to the best value to the government, past performance, instructor credentials, and price will be considered. Only one award will be made off this solicitation. Past performance and instructor credentials, when combined, are significantly more important than cos t or price. Simplified Acquisition Procedures will be utilized. This combined solicitation/synopsis can be downloaded from web site www.fedbizops.gov COMBAT LIFESAVER COURSE STATEMENT OF WORK COURSE CONTENT 1. CLS course must be taught to Army standards as outlined in Sub course ISO873, Edition B, as published by the Army Institute for Professional Development. 2. Instruction topics will include as a minimum the following nine lesson plans as defined in ISO873: * PERFORMING TACTICAL COMBAT CASUALTY CARE * EVALUATING A CASUALTY * OPENING AND MANAGING A CASUALTYS AIRWAY * TREATING PENETRATING CHEST TRAUMA AND DECOMPRESSING A TENSION PNEUMOTHORAX * CONTROLLING BLEEDING * INITIATING A SALINE LOCK AND INTRAVENOUS (IV) INFUSION * INITIATING A FIELD MEDICAL CARD * REQUESTING MEDICAL EVACUATION * EVACUATING A CASUALTY USING A SKED OR IMPROVISED LITTER Reference materials are: AR 350-1 DA PAM 350-59 Task: Conduct Combat Lifesaver (CLS) Certification Condition: Given a 40 hour course, qualified CLS instructors, actual medical supplies from a serviceable CLS bag, a classroom and field training environment, a complete written comprehensive multiple-choice examination, a written performance examination f or the field medical card and MEDEVAC request, and all hands-on examinations for combat lifesaver certification and recertification. Instructions are conducted within the AMEDD standards for Combat Lifesaver certification. Soldiers that certified prior t o September 2005 must retake the entire certification course due to the new course standards. The uniform for classroom instruction is duty uniform and the uniform for hands on evaluation is full combat uniform to include BDUs, ACUs, or DCUs, Kevlar or ad vanced combat helmet, body armor (outer tactical vest (OTV) with SAPI plates), neck protector, throat protector, groin protector, ballistic eye protection, knee and elbow pads, gloves, combat earplugs and assigned personal weapon. Standard: Soldiers will score a minimum of 70 percent on the written examination and score a GO on each written performance examination in order to complete this course. A NO-GO on any step of a performance checklist or hands-on examination will result i n a NO-GO for the entire checklist for combat lifesaver certification and recertification. INSTRUCTORS 1. Course instructors will possess one of the following qualifications: * Primary MOS of 68W (BNCOC graduate) or 18D * State or federally licensed: * Paramedic (EMT-P) * Registered nurse (RN) * Physician assistant (PA) * Nurse practitioner (NP) * Medical doctor (MD) *Vendors must furnish the credentials of the instructors that will be teaching in North Dakota along with their proposal. 2. Instructor to student ratios will be a minimum of one (1) instructor per six (6) students for any of the hands on training tasks and one (1) instructor per twenty-four (24) students for the classroom instruction. 3. The North Dakota National Guard will be responsible for enrolling students in the Combat Lifesaver Course with the Army Institute for Professional Development (AIPD) prior to the course, and submission of a course roster to AIPD upon completion. Soldie rs will also be enrolled on the Army Training Requirements and Resources System (ATRRS) by the North Dakota National Guard. TIMELINE 1. Each class with a maximum of 24 students will be conducted over a 3 day period and include at a minimum, 26 hours of instruction per course POI standards. This may very well be extended up to the maximum 40 hours dependent upon how quickly the soldiers grasp the subject matter and are able to perform each of the required tasks. Every soldier enrolled must be trained to standard and finish the course as a qualified Combat Lifesaver. 2. Training will begin on the first day of each course at 0800. Subsequent training days will begin and end based on training requirements as determined by the previous days schedule. Daily end times will be determined by successful completion of daily schedule. EQUIPMENT/FACILITIES 1. The NDARNG will provide the student manuals (ISO871), instructor manuals (ISO873), and the following Class VIII medical supplies for the courses. All training aids and other equipment will be supplied by the course instructors. 2. The NDARNG will provide a classroom suitable for 24 students and 4 instructors. The classrooms will at a minimum include chalkboard/whiteboard and projector for slideshows/videos. The Armory drill floor (basketball court size) is also available for a ny practical exercise portion of the course at both locations. The January weather in ND is normally not suitable for this type of training to be conducted outdoors. CLASS VIII MEDICAL SUPPLY 1. This list includes the consumable materials that will be provided for the courses by the NDARNG: Field dressings Cravats Examination gloves Chucks 18 gauge IV needles 14 gauge IV needles Sharps containers Trash bags (including Biohazard bags) IV administration sets 250 mL bags NaCl Rolls medical tape Band-Aids 2 x 2 gauze sponges 4 x 4 gauge sponges Provo done pads Alcohol pads Oropharyngeal airways HemCon training dressings Asherman chest seals Quick-clot ACS Combat Application Tourniquets (CAT) The vendor who is awarded the contract must provide adequate completion documentation in the form of a diploma or completion certificate to every soldier who completes the training. At a minimum the diploma should include the date the training was conduct ed and state that the individual (name typed on Diploma) attended 40 hrs of training and successfully completed the Combat Lifesavers Course IAW Sub course ISO873. Please include a listing of all of your past performances with other military or comparable organizations that you have conducted CLS or similar training for in the past three to five years to include points of contacts for references along with your quota tion. Ensure your packet includes instructor credentials, past performance, references, and pricing information.
