Description
DESCRIPTION: This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. Solicitation Number VA-250-08-RP-0098 is being issued as Request for Quote (RFP). The solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-22. This requirement is a 100% small business set aside under NAICS Code 562112, Size Standard of $11.5 million dollars. REQUIREMENTS: The Louis Stokes Cleveland Department of Veterans Affairs Medical Center (VAMC), 1000 Brecksville Road, Brecksville, Ohio 44141 (hereafter known as the VAMC), has an emergency requirement for the disposal of Radioactive waste. The following radioactive waste needs to be packed, sealed, posted/labeled as appropriate, weighed, solidified in concrete as appropriate, manifests prepared, and transported for treatment/burial/disposal as appropriate: 42 tritium signs , 9 55-gallon drums of solid un-compacted wastes each drum assumed to be 125 lbs , 2 55 gallon drums of deregulated scintillation vials, 9 Cobalt-57 flood sources, 2 liters of Uranyl-238 Acetate to be solidified in concrete and contained within a 10-gallon pail. Pick and Disposal of all radioactive materials must be disposed of with two days of award. Quotes will be due Wednesday, December 12, 2007 by 4:30 pm EST. Only Electronic quotes will be accepted. Quotes shall be emailed to Daniel.Rakes@va.gov. PRICING AND LINE ITEMS: Price shall include all items listed above CONTRACT PROVISIONS AND CLAUSES: Contract clauses and provisions cited in this solicitation may be accessed electronically at these addresses: http://www.arnet.gov/far/ and http://www.va.gov/oamm/acquisitions/ars/policyreg/vaar/vaar852.htm . In accordance with FAR 52.204.7 - Central Contractor Registration (Oct 2003); contractors are Required To Register in the Central Contractor Registration (CCR) database http://www.ccr.gov, PRIOR to award of any contract. FAR 52.212-1, Instructions to Offerors- Commercial, applies to this acquisition. The following clause applies to this acquisition: FAR 52.212-2 Evaluation-Commercial Items. Evaluation criteria will be based on price, technical capabilities, and past performance. Vendors shall provide at least two references of past performance with the quote. Evaluation may be made with or without discussions with the offeror(s). Award will be based on overall best value to the Government. Offerors are to include a completed copy of the provision at 52.212-3, Offeror Representations and Certifications-Commercial Items with its offer. 52.212-4, Contract Terms and Conditions-Commercial Items applies to this acquisition and the following addenda: VAAR 852.211-77. 52.212-5, Contract Terms and Conditions Required To Implement Statutes Or Executive Orders-Commercial Items applies to this acquisition. OFFERS: All responsible sources that meet the VAMC requirements may respond to this solicitation by submitting a quote that provides; (1) Documentation (as listed above), (2) Pricing (3) Response to Evaluation Factors. Any offer that does not meet the solicitation requirements as deemed by the Contracting Officer may be rejected as non-responsive. All offers must provide company name, point of contact information and the VAMC solicitation number. No phone calls will be accepted.
