Description
Amendment 2 issued to add the Business Declaration Form as an attachment. The Business Declaration form is to be included with each submittal. Amendment 1 issued 12/12/07 to add spreadsheet with information regarding building types and square footage. The Federal Aviation Administration (FAA) at the Mike Monroney Aeronautical Center (MMAC), Oklahoma City, OK is seeking information about conducting an arc flash hazard assessment, e.g. assessment of facility and equipment to determine the arc flash protection boundary and the personal protection equipment that people with the flash protection boundary shall use. The FAA requests electrical hazard assessment firms to provide information to estimate the project cost and time requirements. The FAA is interested in identifying companies who have previously conducted arc flash assessments at comparable facility types. The MMAC's assessment will include more than 60 buildings; including both industrial (40) and office (20) buildings. Service voltage supplied to the facilities is 12470 VAC. Facilities primarily utilize 480 VAC; however, various voltage levels exist. Personnel conducting electrical hazard assessment should have appropriate technical qualifications, as stated in 29 CFR 1910.332 Qualified Person. Arc flash hazard assessments will likely include: ?? Conducting assessments in accordance with standards, including IEEE 1584 and NFPA 70E ?? Gathering of nameplate and electrical data with equipment energized ?? Analysis of equipment down to 50 volts or 0 hazard categories as necessary ?? Conducting short-circuit current flow and coordination based on various loads ?? Providing updated one-line diagrams that are compatible with AutoCAD 2007 ?? Determining arc flash hazards related to facilities and equipment ?? Providing recommendations to lower hazard categories ?? Producing detailed labels to be affixed to equipment ?? Conducting associated employee training for arc flash hazards ?? Providing documents utilized during the hazard assessment ?? Providing software used to calculate arc flash Respondents to this market survey are requested to provide the following documentation for FAA review. ?? Past performance references for contracts at comparable facilities. References, no more than three (3), shall be provided for contracts performed in the past three years. The list of references shall include: (1) Customer name and address; (2) Contract number and date awarded; (3) Name, address, e-mail address (if known), telephone and FAX numbers for the government Contracting Officer (procuring or administrative); for non-government contracts, provide the name, address, e-mail address (if known), telephone and FAX numbers of business point of contact, liaison, etc.; and (4) Description of assessment performed and facility type and size. ?? Rough Order of Magnitude cost estimate and project plan ?? Timeline for the proposed project ?? Business Declaration Form, attached Three (3) copies of the information required above shall be provided with each offeror's submittal. Submittals shall not exceed twenty (20) pages in length. The results of this market survey are for internal use only. They will be used to determine whether an external contract should be pursued or if the work should be performed internally. This market survey may not result in a contract. Additionally, this market survey will assist the FAA in determining if a future contract will be set-aside for small or Socially and Economically Disadvantaged (8a) small businesses. Technical documentation should be received no later than 2:00 p.m. CST, January 7, 2008. Technical documentation shall be addressed to FAA, Customer Service Desk (AMQ-100), 6500 S. MacArthur Boulevard, P.O. Box 25082, Oklahoma City, Oklahoma 73125. Hand-carried proposals (or proposals sent by overnight delivery) shall be delivered to Room 313 of the Multi-Purpose Building located at 6500 South MacArthur Boulevard, Oklahoma City, Oklahoma 73169. Submittals must be sealed and labeled as follows: Attn: Connie Houpt, Arc Flash Market Survey, due 2:00 p.m. CST, January 7, 2008. Hand-delivered responses will need to allow sufficient time to process through security procedures in place at the Aeronautical Center. Overnight delivery of responses to this market survey could also be impacted if not sent in sufficient time to allow for special mail handling procedures in place at the Aeronautical Center. This market survey is not to be construed as a contract or a commitment of any kind. The Government shall not be liable for payment of any costs attributed to the preparation and submission of requested technical documentation.
