COMBINED SYNOPSIS / SOLICITATION (i) This is a combined synopsis/solicitation for commercial items in accordance with the format in FAR Subpart 12.6 as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and written solicitation will not be issued. (ii) Request for Quote (RFQ) F3F3CE7324AQ01is a combined synopsis/solicitation for commercial items. This combined synopsis/solicitation is issued as a RFQ. Submit written offers only, oral offers will not be accepted. All firms or individuals responding must be registered with the Central Contractor Registration (CCR). (iii) This combined synopsis/solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition Circular 2005-21 and through Department of Defense Acquisition Regulation Change Notice 20071207. It is the contractor?s responsibility to be familiar with applicable clauses and provisions. Clauses and provisions can be found at: http://farsite.hill.af.mil/vffar1.htm (iv) This procurement is being issued as 100% Small Business Set-Aside. North American Industrial Classification Standard: 562998; Small Business Size Standard: $6.5M (v) CLIN (0001) Inspect, pump, and remove approximately 10,000 gallons of product from 8 grease traps that are located in 8 different facilities on Mountain Home AFB. Service will be performed on a quarterly basis IAW Statement of Work. Base year with 4 option years. **Please include pricing on each individual option year** (vi) The Government will award a firm-fixed price contract resulting from this combined synopsis/solicitation to the responsible offeror whose offer conforming to the synopsis/solicitation will be most advantageous to the Government, price and other factors considered. Award shall be based on All or None and Best Value to the Government. Best value can include but is not limited to price, delivery date, and technical capability. Quotes are required to be received no later than 16:30 MST, 28 December 2007. STATEMENT OF WORK AIR COMBAT COMMAND GREASE TRAP MAINTENANCE DECEMBER 2007 1. INTRODUCTION In accordance with the basic contract statement of work, the contractor shall accomplish all actions necessary to accomplish on a quarterly basis, pumping, inspection and removal of product of all grease traps identified by 366 CES/CEOUW, as defined by the requirements of this statement of work. 1.1 GENERAL. This statement of work (SOW) defines requirements for accomplishing a quarterly inspection, pumping and removal of product for 366 CES/CEOUW, MHAFB, Idaho. Contractor shall provide all labor, services and materials required to inspect, pump and remove product from MHAFB, on all grease traps identified by CEOUW as needing serviced. All removed waste must be hauled off the confines of Mountain Home AFB and disposed of at an approved receiving site, identified by contractor. Contractor will provide MHAFB with site disposal approval certification. The goal of this effort is to ensure the base?s compliance with all pertinent state and federal regulations. First on site visit will be NLT 31 January 2008. 1.2 BACKGROUND Grease trap pumping is an integral part of the utilities section recurring work program (RWP). Current wastewater treatment facility doesn?t treat fats/oils/and grease (FOG) products and may result in permit violations. 1.3 SCOPE The contractor shall furnish the personnel, equipment, materials and other requirements necessary for, or incidental to the performance of work set forth herein. Contractor shall inspect all identified grease traps for proper operation, pump as necessary and remove pumped product from MHAFB for proper disposal. No chemical or biological additives will be dumped into the grease traps without prior approval from MHAFB and will be included in the contract cost. Contractor shall perform this service only after being contacted by a 366 CES/CEOUW representitive for designated locations during the term of this contract. Only remove the solids/grease, not the entire tank volume, liquids can remain. 366 CES/CEOUW will determine the need for entire tank pumping. 1.3.1. PAYMENT/INVOICING The contractor shall be paid by method of Government Purchase Card, Convience Check. The contractor shall only be paid for services rendered. 1.4 APPLICABLE LAWS AND REGULATIONS The contractor shall comply with all applicable federal, state, local and Department of Defense (DoD) statutes, instructions, manuals, handbooks, regulations and rules (including all changes and amendments). Primary laws, regulations and references the contractor shall comply with include but are not limited to: ? Uniform Plumbing Code (UPC) ? National Standard Plumbing Code ? AFI 32-1066, Plumbing Systems 2.0 PROJECT ADMINISTRATION AND MANAGEMENT 2.1 GOVERNMENT FURNISHED MATERIAL Base support MHAFB, 366th CES/CEOUW shall provide the following ? Grease trap location maps ? Necessary access to all buildings and applicable areas (e.g., mechanical rooms) either by authorized escorts or by furnished keys and or lock combinations 2.2 GOVERNMENT POINTS OF CONTACT Government points of contact are: ?
