- This is a Sources Sought Notice only. This is not a Request for Quotes/Proposals. A solicitation is not available. The tracking number for this announcement is VA-258-08-RI-0033. The Government will use responses to this Sources Sought Notice to make appropriate acquisition decisions regarding the type of procurement to be utilized to obtain the supplies and equipment. The purpose of this Sources Sought Notice is to identify potential Service-Disabled Veteran-Owned Small Business (SDVOSB), Veteran-Owned Small Business (VOSB, and other Small Business sources that can provide the following for the Southern AZ VA Health Care Systems (SAVAHCS): The cost-free rental of a Medical Equipment Dry Acid Concentrate Mixer (powder) for dialysis. Multi-formula, 50 to 100 Gallon mixing tank must be 510(k), FDA Approved, GPA and have the ability to receive direct Reverse Osmosis feed and pump into a 250 gallon holding tank. The equipment shall be put into service and remain on-site in the Hemodialysis section of the SAVAHCS. SAVAHCS expects to purchase the variable quantities of disposables listed below, which are used in conjunction with the equipment during the contract period. Dry Mix Acid Concentrate Dialysate 2K+ 2.5CA++ Dry Mix Acid Concentrate Dialysate 3K+ 2.5CA++ All eligible SDVOSB, VOSB, & other Small Business firms responding to this Sources Sought Notice must ensure their response is complete and sufficiently detailed to allow the Government to determine the firms' qualifications to perform this requirement. Interested firms must provide a Capability Statement with the following information: Company Name, Address, Point of Contact, Telephone and Fax Number, Email Address, Business Size, and DUNS Number. Past Performance: List your three most recent contract awards for like or similar work, to include Client's Name, Point of Contact, Phone Number, Description of Services, Dollar Amount, and Period of Performance. This market research does not constitute an invitation for bid or request for proposal and is not to be construed as a commitment by the Government. The Government will not reimburse firms for any cost incurred for their participation as an interested party. All responses must be submitted by email to Lillian.Provencio@va.gov; no other method of transmittal will be accepted. All capability statements and questions must be received by Feb 1, 2008. Telephonic requests will not be acceptable. The North American Industry Classification System (NAICS) code is 334510, and the Small Business Size Standard is 500 Employees.
