Description

This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued.Solicitation W81NWY80098499 is issued as a request for quotation (RFQ). This solicitation document and incorporated provisions and clauses are those in effect through Federal Acquisition C ircular 2005-05. This acquisition is unrestricted under NAICS code 339112 and the small business size standard is 500 employees. The requirements in this solicitation are: CLIN 0001, 2 each MAYFIELD ULTRA 360 PATIENT POSITIONING SYSTEM, CAT#A2009TROL, a nd CLIN 0002, 2 each MODIFIED MAYFIELD SKULL CLAMP, CAT#A1059. Federal Acquisition Regulation (FAR) Clause 52.211-6 Brand Name or Equal applies to this acquisition. CLIN 0001 salient characteristics are: Independent rotating and self locking handles prov ide more fluid and flexible movement allowing better connection to the desired skull clamp position. Easier, more fluid movement for quicker positioning with skull clamp. Two double-cam action locking handles for easier opening and closing for secure, qu ick fixation. Set-up time is reduced because of the quick action handles. More flexible movement with added rotation for degrees of freedom. Unit can be placed in the reverse position to allow more reach and added height. Shall be compatible with MAYFIE LD HEADREST A-1012. CLIN 0002 salient characteristics are: Provides 3-point righid cranial fixation. Flexibility and safety in positioning of skull pins around critical areas. Quick-release lock enables easy removal. Shall be compatible with MAYFIELD H EADREST A-1012. Vendors shall comply with FAR 52.211-6 if submitting other than the Brand Name product. Clause 52.211-6 (and other clauses throughout this solicitation) can be viewed by accessing website http://farsite.hill.af.mil/ . Delivery, 30 days after receipt of order, shipping shall be FOB destination to Brooke Army Medical Center, 3851 Roger Brooke, Receiving Dock, Fort Sam Houston, Texas 78234-6200. FAR provision 52.212-1 [Instructions to Offerors  Commercial] applies to this acquisition to include the following addenda: All offerors are cautioned that if selected for award, they must be registered with the Central Contractor Registration (CCR) program. The CCR can be accessed via the internet at www.ccr.gov/ . Confirmation of CCR regis tration will be validated prior to awarding a contract. Evaluation Process: All proposals will be evaluated in accordance with the following criteria: Lowest Priced, Technically Acceptable. Technical Capability (compliance with the requirements of FAR Clause 52.211-6 if vendor offers and Equal Product). Vendors shall include a completed copy of FAR provision 52.212-3 with their offer. The provision can be downloaded by accessing the website listed in this solicitation.
