- FAMILY HEALTH CENTERS AS EXTENSIONS OF DEWITT HEALTH CARE NETWORK, FORT BELVOIR, VIRGINIA. This requirement is to provide continued community based Family Health Centers, which are designated as primary care entry points for authorized military bene ficiaries of the Military Healthcare System and DeWitt Health Care Network located in the southern corridor of the National Capital Area. The family health centers shall provide the full range of primary care services, child/adolescent and adult mental he alth services, optometry services, and other limited specialty care services as set forth in the performance work statement. The offerors will be responsible for designing, providing and operating two medical clinics in the general vicinities of Fairfax a nd Woodbridge, Virginia. The two current Family Health Centers provide community based primary care and some specialty care for a current combined enrolled beneficiary population of 47,000, which generate over 17,000 clinic visits per month and 49,000 pha rmacy prescriptions per month. The centers will also make limited space available for care of non-enrolled beneficiaries. We encourage small businesses to reply with evidence of their capabilities in leasing facilities, medical build-out and providing th e above services. Request interested parties provide information as to their capabilities and experience in providing similar services to the points of contact listed below not later than 28 January 2008 in order for the Government to develop a course of action. The transition period is anticipated to begin 1 October 2008 with full performance anticipated to begin 1 April 2009 with four (4) one-year options.
