- REAGENT KITS. The Air Force Institute for Operational Health, Epidemiological Surveillance Division (AFIOH/SDE) intends to award Sole Source, Firm-Fixed Priced contract to Gen-Probe, 10210 Genetic Center Drive, San Diego, CA 92121 for in-vitro molecular diagnostic instrumentation, training and reagents for Chlamydia trachomatis and Neisseria gonorroheae testing using Nucleic Acid Amplification technology. This is a COMBINED SYNOPSIS/SOLICITATION for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; quotations are being requested and a written solicitation will not be issued. PROPOSALS SHALL BE VALID FOR 90 DAYS. This solicitation document incorporates provisions and clauses in effect through Federal Acquisition Circular 2005-18. The associated NAICS code is 325413, and the small business size standard is 500 employees. Estimated Yearly Quantities: Combo Assay Reagent Kits is 192 each, GC verification kits (250 is 1 each, CT verification kit (250 is 22 each, GC verification kits (100 is 52 each, CT verification kit (100 is 50 each, Urine Specimen Collection Kit (100 is 100 each, and Unisex Swab Collection Kit (100 is 100 each. FOB: Destination. AFIOH/SDE, 2513 Kennedy Circle, Brooks City-Base, TX 78235. This effort will result in an ID/IQ contract with a base year and one option year.
