- Manchester VAMC 718 Smyth Road Manchester, NH 03104 Position Description - Phlebotomist Somersworth Community Based Outpatient Clinic (CBOC) Major Duties " Performs accessioning function and data entry " Performs phlebotomy, maintains phlebotomy supplies and stores supplies in proper area at Somersworth CBOC location " Performs Technical work in support of diagnostic tests and procedures for Somersworth CBOC location. " Operates and maintains instruments, equipment and supplies specific for diagnostic procedures. " Maintains monitoring of centrifuge equipment per Primary Care Infection Control Policy 11C-06-009. " In the performance of official duties the incumbent has regular access to printed and electronic files containing sensitive data which must be protected under the provisions of the Privacy Act of 1974 and other applicable laws, federal regulations, VA statutes and policy, and VHA policy. Sets up Equipment, Determines Technical factors, and sets controls " Determines technical factors, sets controls and operates equipment and instruments such as centrifuges. The incumbent sets up specialized equipment for routine procedures (centrifuges), sets technical factors and adjusts for patient specimens and testing ordered. Reports instrument malfunctions to CBOC Nurse Manager/Lab Manager. Recognizes abnormalities and reports to CBOC Nurse Manager/Lab Manager or Pathologist. Operates equipment and performs diagnostic procedures " Performs procedures such as phlebotomy, used to diagnose medical conditions with expertise. " Operates and maintains technical equipment and accessories used by Manchester's Pathology & Laboratory Medical Service (P&LMS) to assist with or perform standard diagnostic or therapeutic procedures requiring adjustments for abnormalities. " Positions patients in standard postures for proper phlebotomy, verifies patient identification using two patient identifiers (Full name and full SSN) and explains sequence of events to patient before performing procedure. Follows a step by step process to assure correct operation of equipment and specimens drawn are acceptable. " Provides limited orientation to staff (as necessary) regarding equipment or instruments used by the position. " Answers telephone calls related to lab testing at Somersworth CBOC location and functions as liaison for clinic personnel in executing the order entry and results review options on VISTA. " Performs direct patient support work to include collecting information from patients, providing personal patient care and educating patients on pre-testing and post-testing activities such as phlebotomy and glucometer usage. Maintains Control Records and Documents Test Results " Performs administrative tasks such as maintaining control records for specimens and tests " Recognizes emergency situations and establishes priorities for emergency orders. Reports changes in veteran's health care status to CBOC personnel. " Obtains and provides basic medical information for patients and caregivers. " Verifies laboratory orders, clarifies unusual requests with initiating practitioner, confirms proper patient collection specimens, accessions specimens into P&LMS computer package and packages testing specimens according to established guidelines to ensure safe transportation of specimens. " Provides basic patient education on pre-testing and post-testing patient activities for specialized testing such as glucose tolerances. " Performs basic work in CBOC settings to include phlebotomy and centrifuging and packaging patient specimens for transportation to Manchester VAMC or other facilities as indicated. " Performs work involving tests and examinations using the techniques of medical laboratory practice in one of more specialized areas. Applies a practical knowledge of the chemistry, biological and anatomy involved, e.g. for phlebotomy - use of specialized collection techniques and equipment for difficult draws, correct tubes for collection, timing requirements for certain specialized test requests. " Performs work requiring a practical knowledge of the methods and techniques of one or more of biological or physical sciences providing support in a medical laboratory. E.g. glucometer correlations. Maintains Equipment/Supplies/Facilities " Maintains the Somersworth CBOC laboratory and equipment. Requests needed supplies. " Adheres to all safety regulations and has knowledge of proper infection control practices (universal precautions) based on CDC guidelines and Medical Center infection control policies and procedures. " Monitors specimens, equipment and the environment for contamination. Performs all cleaning and disinfecting of areas as required. Identifies any contaminations noticed to the CBOC Nurse Manager/Lab Manager and recommends corrective action. " Assists in the inventory and distributions of various laboratory supplies or commercial media. Processes requisitions, receives and stores supplies noting damage, contamination, hemolysis, shortage or other errors in shipment. Maintains Laboratory Records " Maintains and controls laboratory records for phlebotomy, supplies inventory. " Maintains records of results and related documents. Prepares/Examines Specimens " Accepts into CBOC laboratory all specimens, spins down appropriate specimens and readies them for pick up and delivery based on established pick up timeframes. " Follows standard procedures and applies a practical knowledge of the properties of the specimens to ensure effective presentation of the qualities and characteristics critical for accurate testing or examination. " Prepares samples, slides or other types of specimens (i.e. blood, urine, tissue) common to the laboratory. Knowledge required by the position " Practical knowledge of standard procedures using any one of several technical procedures. " Skill in the operations and use of specialized equipment and materials " Excellent internal and external customer service skills. Supervisory controls " Works independently in Somersworth CBOC setting. " Supervised by the Community Based Outpatient Clinic (CBOC) Nurse Manager/designee. " Secondary oversight is provided by the Lab Manager/designee in conjunction with the CBOC Nurse Manager. Guidelines " Specific, detailed guidelines (procedure manuals) covering all important aspects of the assignment are provided by the Lab Manager. The employee adheres strictly to the guidelines; deviations must be authorized by the Lab Manager. Complexity " The work follows a sequence of steps or processes that the employee can follow without supervisory assistance. Decisions regarding what needs to be done involve various choices requiring the employee to recognize the differences among a few easily recognizable, specified alternatives. Scope and Effect " The work involves the performance of a variety of routine, standardized tasks which facilitate work performed by higher level employees and contributes to a base of patient care provided by the CBOC and main medical facility. Personal Contacts " Contacts are primarily with personnel within the CBOC and with patients. " Frequent contact and collaboration of care with Anticoagulant Clinic personnel. " Other contacts are with Manchester lab personnel for direction in lab test requirements. Purpose of contacts " Contacts with CBOC personnel are for the purpose of giving and receiving information that is necessary for the accomplishment of the work. Physical demands " The work is primarily sedentary but also requires some standing, walking, bending, lifting and carrying of light items. " The work requires above average agility and dexterity. " The work may require heavy lifting of equipment or patients. Work Environment " The work environment involves risks typically associated with performance facility work. Work involves regular and recurring exposure to moderate risks or discomforts which require special safety precautions. Days and Times of Phlebotomy Operation: The days of clinic operations are as follows: " Somersworth CBOC - Monday, Wednesday and Friday from 8 AM - 11 AM Any change to these days or times will be done after direct communication and advance notification with the contracting lab service. Leave Coverage: Phlebotomist must make provisions for lab coverage prior to taking planned leave. Phlebotomist must work with CBOC Nurse Manager, Anticoagulant Clinic clinicians and CBOC staff to ensure that the needs of the veterans are accommodated. Annual Training Requirements: Phlebotomist will be expected to take and adhere to Manchester VA Medical Center's mandatory training requirements that are identified which include but are not limited to: " Infection Control " Fire & Safety " Cyber Security " HIPAA - Privacy Training Phlebotomist will undergo initial competency evaluation during orientation. Additional training may be provided during the term of the contract if indicated. Resignation: Phlebotomist must give, at a minimum, 2 weeks notice prior to resignation so that plans can be made to accommodate the laboratory needs of the veterans. Initial Orientation: Initial orientation will take place at the Manchester VA Medical Center (minimum of one day of training). Orientation to Somersworth CBOC will be provided under the direction of CBOC staff.
