- *** The purpose of this amendment is to change the deliver by date from 29 February 2008 to 22 February 2008. The change is due to a schedule conflict with the barge transport and expected start of clinic refurbishment. The barge is required to be delivered to NH Jacksonville 22 February 2008. There are no further changes by this amendment.*** This is a combined synopsis/solicitation for commercial items prepared in accordance with FAR Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation and a written solicitation will not be issued. The Request for Proposal number is N62645-08-R-0004. Proposals are due not later than 3:30 P.M. EST on 05 FEB 2008. Provisions and clauses in effect through Federal Acquisition Circular 2005-23 are incorporated. NAICS 339114, with a Size Standard of 500 employees for reporting actions purposes. This is Full and Open Competition requirement; all qualified vendors are encouraged to submit a proposal. The Naval Medical Logistics Command has a requirement to lease a mobile dental operatory space (hereafter called dental space) at Guantanamo Bay, Cuba (GITMO) for a period of 4 months with two 1-month option periods which may be exercised at the government's option. The dental space has minimum requirements of: 1. Two dental operatories including dental chairs, lights, units, cabinets, sink; 2. A capability for producing intraoral radiographs; 3. Capability for sterilization of the instruments; 4. The space shall provide environmental protection to include heating, cooling, lighting, and weather protected environment; 5. Ensure that equipment is properly secured, whether for transport conditions or on-site usage; 6. Water or air lines must not leak; 7. Electrical fixtures meet (or exceed) NEC requirements; 8. Generator to run all systems independent of outside power, if required; 9. Complete water, air, and suction plumbing systems; 10. Sufficient lighting for dental procedures; (Typically, the mobile dental operatory space would be a vehicle, either a bus, RV, truck cab/chassis, or a trailer, but any transportable solutions proposed will be considered.) Pictures (inside and outside views) of proposed unit are mandatory. The dental space unit is required to be delivered to Jacksonville, Florida by noon 22 February 2008 in order to be transported to Guantanamo Bay. Transportation of equipment to the base is via Blount Island Marine Corps Base, Jacksonville, Florida. The dental space will be required to be secured for sea shipping, meaning that all internal products must be packed/packaged so they do not move due to pitch and roll of a ship. The government requires a 4 month lease of unit with two 1-month options. Vendors shall provide a proposal for the following: Vendor shall provide the price for a 4 month lease of a mobile dental operatory space that meets or exceeds the minimum requirements. The 4 month lease may be extended at the option of the government for two 1-month options. The total duration of the lease is not intended to extend beyond 6 months. The price data shall specify the price for 4 months as one lot, with the 1 month options expressed on a monthly basis. Vendor solutions shall identify the following: the clinical capability proposed, the ability to meet the time frame of the requirement for 22 February 2008 in Jacksonville, FL, the price of the solution(s), and pictures of the dental space offered. Vendor shall provide their anticipated plan to delivery the unit to Jacksonville, FL. EVALUATION FACTORS FOR AWARD: This is a Low-Price Technically Acceptable LPTA procurement. The vendor shall meet the following: 1. & 2. and be able to provide the minimum requirements as detailed above. Award will be made to the proposal that is technically acceptable and has the lowest price. 1. Product Features Meeting/Exceeding Minimum Requirements 2. Price (Options will be considered in the Aggregate Total Price Evaluation) The government intends to award without discussions, but reserves the right to conduct discussions. TECHNICAL PROPOSAL INSTRUCTIONS: Offerors are required to provide sufficient technical documentation (including, but not limited to: product literature, specifications, features, pertinent maintenance or operator manuals, and warranty information) to allow for thorough evaluation of proposal. In addition to the characteristics of the dental capacity, the vendor shall provide company details, specifically: Complete company name and address: Fax number: Phone Number: Current and valid Tax Identification Number (TIN): Current and valid CAGE Code: Current and valid DUNS number: Vendor shall be registered in the Central Contractor Registration website, www.ccr.gov, prior to contract award. Are vendor offered solutions available from the GSA Federal Supply Schedule? Please specify if your company a large, small, small woman owned, small veteran owned, disadvantaged, SDVOSB, or 8A business? Offerors are encouraged to submit as many possible solutions as practical for delivery to Guantanamo Bay by the closing of this solicitation. Offers shall provide pricing for the initial period of 4 months from delivery, and shall include options for each additional month of the requirement. FAR Clauses 52.212-4 & 52.212-5 will be incorporated into the resultant contract award. Proposals to this solicitation are due not later than 3:30 PM (Eastern Time) 05 FEB 2008 and shall be sent via email with MS Office, Excel, or Adobe attachments.
