Description
DEPARTMENT OF VETERANS AFFAIRS Veterans Affairs Eastern Kansas Medical Center Leavenworth, Kansas Performance Work Statement (PWS) 1. SCOPE: The contractor shall furnish a Performance Work Statement to include all labor, transportation, and parts necessary to provide prime mover and a qualified licensed driver to pull a Department of Veterans Affairs supplied Mobile Magnetic Resonance Imaging (MRI) system for the VA Eastern Kansas Health Care System (EKHCS) Campus Leavenworth, Kansas. 2. SERVICES TO BE PROVIDED: a. Provide a performance work statement for transport services. The driver shall have a Class A Commercial Drivers License (CDL) with appropriate designation to haul the MRI trailer mounted equipment. The estimated total trailer weight with equipment is 63K lbs. with a rear axel wt. of 40K lbs. The driver must possess the ability to transport and setup mobile MRI unit at specified locations for the EKHCS see the attached schedule. b. Driver should possess a valid Cardio Pulmonary Resuscitation (CPR) certification. c. The contractor will be performed his/her duties before and after normal VA Eastern Kansas Health Care System, Kansas business hours, 7:00 AM - 5:00 PM, Monday-Friday, except Federal Holidays. The VA reserves the right to extend to Saturday???s and later shifts as warranted by increased scan volume. d. Provider must have experience in performing this task and provide past performance and provide current references of this ability. The required documents shall be provided to the Contracting Officer's Technical Representative (COTR) for review. e. Drives gasoline or diesel powered tractor trailer combination to transport and deliver MRI services in a mobile environment through out EKHCS. Facilitates maintenance of tractor and trailer and assists MRI technologists as needed. f. Drives truck to designated locations per attached schedule, prepares coach, powers up equipment and ensures that stairs, doors, ramps are ready for patient use. The driver shall follow all established safety procedures for setup of the equipment including leveling and initiating the computer software. Driver shall establish connectivity to internet Services (IS) applications as well as phone service. The driver shall power down unit which includes preparation of the equipment for transport to and from the designated sites. g. Inspects truck for defects before and after trips and submits report indicating truck/coach condition. h. Maintains driver log according to local and Department of transportation (DOT) I.C. regulations. Observes safety and security at all times, assists MRI technologist as requested. i. WORK ENVIRONMENT : While performing the duties of this job, the employee is regularly exposed to wet and/or humid conditions; moving mechanical parts and outside weather conditions. The employee is can be exposed to high, precarious places; fumes or airborne particles; toxic or caustic chemicals; extreme cold; extreme heat; risk of electrical shock and vibration. The noise level in the work environment is usually loud. j. SPECIAL PHYSICAL DEMANDS: While performing the duties of this job, the employee is regularly required to talk or hear. The employee is frequently required to stand; walk; sit; use hands to finger, handle, or feel; reach with hands and arms; climb or balance and stoop, kneel, crouch, or crawl. Specific vision abilities required by this job include close vision, distance vision, color vision, peripheral vision, depth perception and ability to adjust focus. k. Language Skills: Ability to read and interpret documents such as safety rules, operating and maintenance instructions, and procedure manuals. Ability to write routine reports and correspondence. Ability to speak effectively before groups of customers or employees of organization. 3. TERMS AND CONDITIONS: Driver shall transport the MRI trailer to the following locations using the tentative MRI schedule for the EKHCS MRI trailer: (See attached schedule) Eastern Kansas Health Care System (EKHCS) Locations: Munson Army Heath Center 550 Pope Ave, Fort Leavenworth KS 66027. Colmery O???Neil Veterans Affairs Medical Centers (VAMC) 2200 SW Gage Blvd Topeka, KS 66622. Dwight David Eisenhower (DDE) Veterans Affairs Medical Center (VAMC), 4101 S 4th Street, Leavenworth, KS 66048 4. SPECIAL INSTRUCTIONS: Contractor shall report all issues to the Contracting Officer???s Technical Representative (COTR) Joe Zuchowski RT R MR, MRI Supervisor, Diagnostics Eastern Kansas Health Care System, 913-682-2000 Ext. 52227 while performing work on the VA Eastern Kansas Health Care System Campus. 5. IDENTIFICATION: The contractor???s field service employee shall wear visible identification at all times while on the premises of the VA Medical Center. Smoking is prohibited inside of buildings. Possession of weapons is prohibited. Enclosed containers, including tool kits, shall be subject to search. Violations of the VA regulations may result in citation answerable in the United States (Federal) District Court. 6. VEHICLES: All vehicles will be locked and the keys removed while performing service on the Medical Center???s property. This is intended to protect the contractor???s property and provide for the safety of the Medical Center???s personnel. Vehicle(s) shall be identified. Parking in the appropriate designated parking areas is expected. Information on parking is available from the VA Police. The VA Medical Center will not invalidate or make reimbursement for parking violations of the contractor for any conditions. 7. SAFETY REQUIRMENTS: In the performance of this contract, the contractor shall take such safety precautions as the Contracting Officer or designee may determine to be reasonably necessary to protect the lives and health of the occupants of any building. The Contracting Officer or designee will notify the contractor of any noncompliance with the foregoing provisions and action to be taken.
