- RFP VA 263-08-RP-0130 The VISN 23 Contracting Office intends to acquire for the VA Medical Center, Minneapolis, MN a replacement Hemodialysis Water Treatment System as outlined below. Offeror to provide, in a TURN-KEY INSTALLATION++, for 15 stations, water treatment system components, a bicarbonate and acid distribution system, and manuals in accordance with RFP VA263-08-RP-0130. The following system components are sought: One (1) each hemodialysis water treatment system; One (1) each bicarbonate and acid distribution system; Two (2) Sets Operator's Manuals; and, One (1) Set Service Manuals. SEE solicitation documents for Scope of Work and detailed requirements. New systems must be FDA 501K certified. ++ TURN-KEY INSTALLATION is defined as the complete de-installation and removal of the old equipment, the complete installation of the new equipment and systems, connecting the new systems to Government utilities, as required, to provide the Government with a system that is complete, functional, and ready to utilize on patients, and the restoration of walls, floors and ceilings. The Government will bring utilities into close proximity to the installation site but will not be responsible for the final connections. NAICS 333319, 500 employees. All responsible sources may submit a written proposal which, if timely received, shall be considered by the Government. PROPOSALS MUST BE COMPLETE AND PROVIDE ALL INFORMATION SOUGHT IN THE SOLICITATION DOCUMENTS. Failure to provide full information may cause proposals to be down-rated in the evaluation process. The Government desires TWO COPIES of the proposals, stapled and not otherwise bound. Solicitation documents are available on or about January 31, 2008 and can be downloaded at www.fbo.gov. Offers are due Friday, February 29, 2008 at 4:30 CT. Proposals will be sent (or delivered) to arrive by the date and time due. If hand delivered bring to the VISN 23 Contracting and Purchasing Office (90C), VA Medical Center, Bldg. 68, One Veterans Drive, Minneapolis, MN 55417. Use the same address for U. S. Mail or other delivery systems/methods. Late proposals will be rejected. Proposals must be in the Government's custody by the due date and time. Delivery will be verified.
