- This is a combined synopsis/solicitation for commercial items prepared in accordance with the format in Subpart 12.6, as supplemented with additional information included in this notice. This announcement constitutes the only solicitation; proposals are being requested and a written solicitation will not be issued. The solicitation number is W91YTV-08-T-0020. This acquisition is issued as a Request for Quote (RFQ). The solicitation document and incorporated provisions and clauses are those in effe ct through Federal Acquisition Circular 2005-22. The North American Industry Classification System Code (NAICS) for this procurement is: 561410 This requirement is 100% Small Business Set-Aside. U. S. Army Health Clinic, Fort McPherson, Georgia, has a r equirement for the following: CLIN 0001: 1 EACH, INITIAL SET UP FEE for scanner and workstation system preparation and set-up. UNIT PRICE: $__________, TOTAL CLIN $__________. CLIN 0002: 150,000 EACH, SCANNING OF MEDICAL RECORDS, ap proximately 1,000 medical records to be scanned, and each medical record averages about 125 pages. This constitutes about 125,000 pages and would require a commitment minimum of 100,000 pages and a maximum of 150,000 pages to be scanned. Requirements: The copying corporation will provide personnel, scanning equipment, and workstations to perform the following tasks: 1. Pre-scan document preparation a. Remove documents from folders b. Remove staples, paper clips etc. c. Soften curled or folde d pages 2. Document scanning of medical records 3. Return scanned documents to their folders 4. Validate/QA all images 5. Index medical records by: a. Last Name b. First Name c. Middle Initial d. Social Security Number e. Date of Birth 6. Each medical record will be saved as a multi-page TIFF formatted file 7. Each medical record image will be archived to CD (2 sets) labeled appropriately with index information 8. Review Images with Customer 9. Weekly reports 10. P roject Schedule Assumptions: 1. The corporation will provide a schedule upon award of contract 2. The deliverables for this task will be multi-page TIFF images archived to CD media 3. Each CD will be assigned a unique identifier 4. Each CD will be delivered to the assigned Lawrence Joel Army Health Clinic Technical Representative 5. Delivery of the CDs will be on an agreed upon schedule 6. The corporation will spot check CDs with Lawrence Joel Army Health Clinic Technical Represent ative for approval 7. Lawrence Joel Army Health Clinic will provide space for system setup and task performance 8. Lawrence Joel Army Health Clinic personnel will deliver all records to be imaged to the scanning site 9. All work will be performed at Lawrence Joel Army Health Clinic during normal business hours 10. The corporation will provide weekly updates including scanned records, deliveries, schedules, issues, etc. 11. The corporation will invoice at the end of each month 12. Invoice a mount will be based solely upon the number of images scanned/indexed and delivered 13. Lawrence Joel Army Health Clinic will remit payment no more than 30 days after invoice date 14. All medical record information is on 8.5 x 11 paper 15. All reco rds will be provided as requested in a timely manner to minimize downtime 16. The corporation will supply CD media, CD cases, labels, and label software. UNIT PRICE: $__________, TOTAL CLIN $__________. THIS SOLICITATION MUST BE QUOTED ALL OR NONE. To be delivered to: U. S.
